睾丸静脉介入栓塞治疗精索静脉曲张的应用解剖

摘　　要：目的：为经皮穿刺股静脉介入栓塞睾丸静脉治疗精索静脉曲张提供解剖学基础。方法：解剖测量２０具成年尸体的睾丸静脉和肾静脉等。结果：①髂总静脉汇合处至左、右肾静脉之间的下腔静脉长度分别为 １２．０ ｃｍ和 １１．８ ｃｍ。②左、右肾静脉长度分别为 ７．５ ｃｍ、３．０ ｃｍ；其中点处外径分别为 １３．１ ｍｍ、１１．７ｍｍ。③左肾静脉与下腔静脉之间形成的夹角平均为７７．９°，左侧睾丸静脉汇入点距下腔静脉的间距为３．４ｃｍ，在此处其外径平均为３．５ ｍｍ，左侧睾丸静脉与左肾静脉间形成的夹角平均为 １０５．６°，汇入点在左肾静脉下壁的占５５％（１１侧）；前下壁的占３５％（７侧）；前壁的仅占１０％（２侧）。结论：本文为行睾丸静脉介入栓塞治疗精索静脉曲张术时提供髂总静脉的长度和下腔静脉与肾静脉的夹角，左睾丸静脉内侧肾静脉的长度、外径，左侧睾丸静脉与肾静脉之间的夹角以及汇入点在肾静脉壁的位置等应用解剖学资料。To provide morphological basis for intervenient thrombosis treatment of varicocele. Methods: The testicular and renal veins of 20 adult cadavers were dissected and measured. Results: ①The average lengths from the confluence of the common iliac veins to the left and right renal veins were 12.0 cm and 11. 8 cm respectively. ②The average lengths of the fell and right veins were 7 .5 cm and 3 .0 cm, and the outer diameter at their middle points were 13. 1 mm and 11 .7 mm respectively. ③The average angle between the left renal vein and the inferior venae cave was 77. 9 degree. The left testicular vein joins the renal vein at an angle of 105. 6 degree, where their average outer diameter was 3 .5 mm and the distance to inferior venae cave was 3 .4 cm. 55% (11 cases) of the left testicular veins joined the renal vein at the inferior wall, 35% (7 cases) at the anterior inferior wall, and 10% (2 cases) at the anterior wall Conclusions:This study provies morphological data for intervenient thrombosis treatment of varicocele, including the length of the inferior venae cave from the confluence of the common iliac veins to the renal veins, the angle between the inferior venae cave and the renal vein, the outer diameter of the left testicular vein at its confluence and the distance to the inferior vein cave, the angle between the tesicular vein and the renal vein, and the location where the testicula vein join the renal vein or the inferior venal cave.
