魏氏督脉经手法治疗腰椎间盘突出症临床研究

摘　　要：目的探讨魏氏伤科督脉经手法治疗腰椎间盘突出症的临床疗效及对患者腰部肌肉功能的影响。方法采用电脑随机数字表法,将60例患者随机分为治疗组和对照组,每组30例。治疗组予以督脉经手法治疗,对照组予以美洛昔康比口服及腰椎牵引。两组疗程均为1个月,采用VAS、JOA腰痛评分表及SF-36量表观察患者的疼痛程度、腰椎功能及生活质量变化情况;同时采用表面肌电图仪对患者腰椎两侧肌肉功能进行检测,并分析其平均肌电值(AEMG)和中位频率的斜率(MFs)。结果两组患者治疗后,疼痛程度均明显减轻(P<0.05)。治疗组腰椎功能及生活质量均显著提高(P<0.01),而对照组患者改善不明显。治疗组治疗后较对照组MFs明显下降(P<0.01),而AEMG则明显提高(P<0.01)。结论魏氏督脉经手法能有效减轻腰椎间盘突出症患者的疼痛程度,改善患者的腰椎功能及生活质量,调整患者腰椎两侧肌肉的功能及协调性。Objective To explore the effect of Wei＇ s Du meridian manipulation in treating lumbar intervertebral disc herniation （LIDH） and to evaluate its effect on muscle function. Methods Sixty cases were randomized into treatment group and control group by random number table, 30 cases in each group. The treatment group was treated by Du meridian manipulation, and control group was treated by Mobic and lumbar traction. Before and after treatment for 1 month, the pain degree, lumbar vertebral funetion and QOL were evaluated. The muscle function of lumbar vertebra at both sides was detected, and the AEMG and MFs were analyzed. Results After treatment for 1 month, the pain degree was relieved in both of the groups （ P 〈 0. 05 ）. The lumbar vertebral function and QOL in treatment group were markedly improved （P 〈 0. 01 ）, while no significant improvement was found in control group. No significant difference was found in AEMG and MFs between the two groups before treatment （ P 〉 0.05 ） ; after treatment for 1 month, the MFs in control group was markedly reduced （ P 〈 0.01 ）, and the AEMG was markedly increased （ P 〈 0.01 ）. Conclusion Du meridian manipulation is effective to alleviate the pain, improve the lumbar vertebral function and QOL by regulating the muscle function and coordination.
