腔镜下甲状腺瘤切除术后并发症的护理体会

摘　　要：目的比较腔镜甲状腺瘤切除术和传统开放甲状腺瘤手术疗效及术后并发症。探讨术后并发症的护理方法。方法选择76例甲状腺瘤切除术的患者．随机分为腔镜手术组（29人）和传统手术组（47人），分别在腔镜下行甲状腺肿瘤切除术和在开放手术下行甲状腺瘤切除术。两组均采用全身麻醉。对比两组患者疗效和并发症出现的情况．总结围手术期的护理要点。结果腔镜组患者住院时间和平均手术时间均比传统手术患者长．而平均出血量和切口满意度均优于传统蛆患者（P〈0.005）；传统开放甲状腺瘤手术组发生术后并发症的机率大于腔镜下行甲状腺肿瘤切除术，两组并发症经过积极治疗和护理，患者顺利康复出院。结论甲状腺瘤切除术术后并发症的护理是手术成功的一个重要的环节：腔镜甲状腺瘤切除术和传统开放甲状腺瘤手术疗效及术后并发症均有一定的差别．腔镜甲状腺瘤切除术疗效较好．术后并发症发生率较小。Objective The therapeutic effect were compared of endoscopic thyroidectomy and traditional thyroid excision. The nursing of postoperative complications was studied. Methods 76 cases of thyroid tumor resection were randomly divided into endoscopic thyroidectomy group （29 persons） and traditional thyroid excision group （47）. Both groups were given general anesthesia. Results The endoscopic thyroidectomy group had longer hospitalization time and longer average operative time than traditional surgery. But for the mean blood loss and satisfaction, the endoscopic thyroidectomy group were superior to the traditional thyroid excision group （P 〈0.005）; the traditional thyroid excision group had higher percent of postoperative complications than the endoscopic thyroidectomy group. After active treatment and nursing care, all patients was recovery and discharged .Conclusion The nursing for postoperative complications of thyroidectomy is an important part of the successful operation;There were some differences in therapeutic effect and postoperative complications between the endoscopic thyroidectomy and traditional thyroid excision. The endoscopic thyroidectomy had better therapeutic effect, less incidence of postoperative complications.
