超声造影在胆囊占位性病变行腹腔镜胆囊切除手术中的应用价值

摘　　要：目的评价超声造影(CEUS)在胆囊占位性病变行腹腔镜胆囊切除术(LC)中的应用价值。方法对2010年1月至2012年1月的50例LC手术胆囊占位性病变患者的超声造影结果及病理诊断结果进行回顾性分析。结果 LC手术时间14～45min,平均住院(2.96±1.33)d。50例患者中,3例为胆囊癌,47例为良性病变,其中胆固醇性息肉、胆囊腺瘤、胆囊腺肌增生症和慢性胆囊炎伴淋巴滤泡增生分别占良性病变的72.34%(34/47)、6.38%(3/47)、21.28%(10/47)和4.26%(2/47)。超声造影诊断为良性病变者与病理诊断符合率为100%(46/46),对胆囊癌的诊断准确率为75%(3/4)。超声造影对胆囊腺肌增生症、胆固醇性息肉和胆囊腺瘤的诊断准确率分别为100%(3/3)、92.86%(13/14)和25%(2/8)。结论胆囊占位性病变患者拟行腹腔镜胆囊切除前建议常规行超声造影检查。Objective To investigate the significance of contrast-enhanced ultrasound（ CEUS ） in diagnosis of occupying lesions in gallbladder treated by laparoscopic cholecystectomy（ LC ）. Methods Results from contrast-enhanced ultrasound and pathologic diagnosis of 50 patients with occupying lesions in gallbladder and treated by LC from January 2010 to January 2012 were retrospectively analyzed in present study. Results Operation time of LC was 14～45 min and average of length of stays was 2.96 ± 1.33 days.Of the 50 patients, 3 had gallbladder cancer and 47 had benign lesion which included 72.34 %（34/47） for cholesterol polyp, 6.38 %（3/47） for adenoma, 21.28%（10/47） for adenomyomatosis and 4.26%（2/47） for chronic cholecystitis. The diagnostic accuracy of CEUS in benign lesion of gallbladder was 100%（46/46） and that was 75%（3/4） in gallbladder cancer in comparison with pathological diagnosis. Diagnostic accuracy of CEUS was 100%（3/3） in adenomyomatosis, 92.86%（13/14） in cholesterol polyp, 25%（2/8） in adenoma, respectively. Conclusions Contrast-enhanced ultrasonography should be routinely used in diagnosis of occupying lesions in gallbladder before LC.
