腹腔镜下全层缝合法行输卵管吻合术的可行性探讨

摘　　要：目的评估腹腔镜下全层缝合法行输卵管吻合术的临床价值。方法回顾性分析2009年1月至2011年12月因输卵管节育术后要求行输卵管吻合术的患者86例,根据手术方式分为腹腔镜下全层缝合行输卵管吻合19例(腹腔镜组)和开腹输卵管显微吻合67例(开腹组)。观察两组手术时间、术中出血量、复通率及术后妊娠率、异位妊娠率等。结果全部病例均顺利完成输卵管吻合术,腹腔镜组与开腹组相比,手术时间短、术中出血少、平均住院日短,但其术后妊娠率与异位妊娠率与开腹组相近,腹腔镜组术后1年的复通率较开腹组高,但结果无统计学意义。结论腹腔镜下全层缝合法行输卵管吻合术是一种安全、可行、临床效果好的微创术式。Objective To evaluate the clinical value of whole-layered suturing anastomosis of tube under laparoscope. Methods Retrospective analysis of 86 cases for anastomosis of tube from January 2009 to December 2011. They were divided into whole-layered suturing anastomosis of tube under laparoscope （ n = 19 ） group and open （ n = 67 ） surgery group. The operation time, blood loss volume, recovery rate, pregnancy rate and ectopic pregnancy rate were measured. Results All the patients finished the anastomosis of the tube successfully. Compared to the open surgery group, we found the laparoscopic group is shorter operation time, less blood loss volume and similar post-operation pregnancy rate and recovery rate. Conclusions Whole-layered suturing anastomosis of tube under laparoscope is safe, feasible and effective micro-invasive operation.
