硬膜外注药结合功能锻炼治疗腰椎间盘突出症患者疼痛的疗效分析

摘　　要：目的:通过硬膜外注药结合功能锻炼治疗腰椎间盘突出症患者,分析其对疼痛的疗效。方法:近3年来我院治疗腰椎间盘突出症患者203例,对腰椎间盘突出症患者使用倍他米松、维生素B1、维生素B12和利多卡因、复方丹参注射液的混合液25mL注入硬膜外腔,结合推拿以及腰背肌的功能锻炼进行治疗。结果:146例腰间盘突出症患者在接受硬膜外给药以后能继续坚持功能锻炼,治愈105例,治愈率71.9%。57例腰间盘突出症患者接受硬模外给药后未能坚持功能锻炼,治愈30例,治愈率52.7%,两者之间的差异有统计学意义(P<0.05)。硬膜外注药结合功能锻炼对轻度和中度腰间盘突出症患者疼痛效果最佳,治愈率可达84.4%,与重度腰间盘突出症患者相比,差异有统计学意义(P<0.05)。结论:硬膜外注药结合功能锻炼治疗轻度和中度腰椎间盘突出症患者疼痛有较好的疗效。Objective： Through the epidural injection drug combined with functional exercise therapy,to analyze clinical therapeutic effect on lumbar disc herniation pain.Methods： 203 cases of lumbar disc herniation accepted treatments in our hospital in the recent 3 years.25 ml miscible liquids composed of glucocorticoid,vitamin B1,vitamin B12,lidocaine and compound salvia miltiorrhiza liquids were injected into lumbar epidural space of patients with lumbar disc herniation pain,then they received manipulation and lumbar back muscle function exercise.Results： 146 cases of lumbar disc herniation can insist to functional exercise after undergoing epidural drug.105 cases of that was cured and the curative ratio is 71.9%.57 cases of lumbar disc herniation can’t insist to functional exercise after undergoing epidural drug.30 cases of was cured and the curative ratio is 52.7%.The differences between them are statistically significant（P0.05）.The clinical therapeutic effect of epidural injection drug combined with functional exercise therapy on mild and moderate lumbar disc herniation pain is optimum and the curative ratio is 84.4%.Compared with patients with severe lumbar disc herniation pain,the differences are statistically significant（P0.05）.Conclusions： The therapy of epidural injection drug combined with functional exercise can have ideal clinical therapeutic effect on patients with mild and moderate lumbar disc herniation pain.
