摘 要:目的分析厦门市结核分枝杆菌的分子流行病学特征,为肺结核病控制提供科学的分子流行病学等信息。方法选择15个可变数目串联重复位点(Variable number tandem repeats,VNTR)位点,对来源于厦门市的临床分离菌株DNA进行检测,检测结果使用BioNumerics(Version4.5)软件进行聚类分析。结果82株结核分枝杆菌被分为Ⅰ、Ⅱ、Ⅲ、Ⅳ、Ⅴ、Ⅵ等6大基因群,以Ⅰ群为主,包含63(76.8%)个株菌;流动人口结核分枝杆菌基因群为Ⅰ、Ⅱ、Ⅳ、Ⅴ、Ⅵ,常驻人口为Ⅰ、Ⅱ、Ⅲ、Ⅳ、Ⅴ;流动人口与常驻人口的结核分枝杆菌VNTR基因型存在一定差异,但均以Ⅰ群为主。Ⅰ群菌株耐药性与其他基因群的耐药性差异无统计学意义(P>0.05)。结论初步证实厦门市结核分枝杆菌菌株存在明显的基因多态性,流动人口与常驻人口流行株均以Ⅰ群流行为主,应加强此类菌株流行的监控。Objective To analyze the molecular characteristic of Mycobacterium tuberculosis (MTB) from Xiamen city in Fujian province, and provide epidemiological reference for pneumonia tuberculosis control. Methods Fifteen loci VNTR was applied to detect 82 clinical MTB isolates from Xiamen, and cluster analysis of the results were performed by using BioNumerics(Version 4.5). Results The 82 strains were classified to 6 genetic groups containing I, II, III, IV, V, WI with group I as the main group which including 63 strains (76.8%).The genetic groups of MTB from the floating population were I , II, IV, V, WI, while the that for permanent residents were I , II, III, IV and V. There were differences in the genotypes of MTB from the floating population and permanent residents by VNTR all with Group I predominated. No significant difference in drug resistance were observed in isolates of group I and other genotypes (P〉0.05).GOrlCIusion Apparently diversified genotypes exists in isolated MTB isolates in Xianmen and group I was the predominant genotype in the floating population and permanent residents. The monitoring of the MTB isolates be strengthened.
