摘 要:目的用^99Tc^m-甲氧基异丁基异腈(MIBI)门控心肌灌注显像方法评价缺血性心肌病患者自体骨髓干细胞(MSC)移植术的疗效。方法缺血性心肌病患者42例,其中20例为试验组,行常规药物治疗联合自体MSC移植术;拒行自体MSC移植术治疗22例患者作为对照组,行常规药物治疗。治疗后3个月和6个月时进行^99Tc^m—MIBI门控心肌灌注断层显像,通过心肌血流灌注改善程度评价自体MSC移植术疗效。结果患者3个月和6个月时心肌放射性缺损节段数目试验组为2.66+0.94和2.52±0.82,较对照组的5.64±1.56和5.51±1.45明显减少(£=2.43,2.65,P均〈0.05)。与自体MSC移植术治疗前(5.72±1.54)相比,试验组患者3个月和6个月时心肌放射性缺损节段数目(2.66±0.94,2.52±0.82)均明显减少(t=2.37,2.58,P均〈0.05)。与自体MSC移植术治疗前160m相比,试验组患者3个月和6个月时6min步行试验距离(368rn和466m)均明显增加(t=2.55,2.48,P均〈0.05)。结论^99TC^m-MIBI门控心肌灌注显像可评价缺血性心肌病患者自体MSC移植的疗效。Objective The objective of this study was to evaluate the clinical value of ^99Fc^m-methoxy- isobutylisonitrile (MIBI) gated myocardial perfusion imaging in patients with ischemic myocardium treated by mes- enchymal stem cell (MSC) transplantation. Methods Forty-two patients with ischemic myocardium who have ac- cepted standard medication were included. 20 of them underwent MSC transplantation therapy. All the patients were examined by ^99Fc^m-MIBI gated myocardial perfusion imaging at baseline and after therapy to evaluate the left ven- tricular myocardial perfusion. The t-test was used to compare data statistically with SPSS 13.0. Results After 3, 6 months, the number of myocardial segments with perfusion defects decreased in the patients treated by MSC transplantation (2.66±0.94) and (2.52±0.82), compared with the control group without MSC transplantation thera- py (5.64±1.56 ) and (5.51 ±1.45 ) (t =2.43, 2.65, both P〈0.05 ). In the patients treated by MSC transplantation the number of myocardial segments with perfusion defects (5.72+1.54) decreased after MSC transplantation (2.66± 0.94) and (2.52±0.82)(t=2.37, 2.58, both P〈0.05). In the patients treated by MSC transplantation the distance (160 m) increased after MSC transplantation (368 m and 466 m, t=2.55, 2.48, both P〈0.05). Conclusion ^99Fc^m-MIBI gated myocardial perfusion imaging is an effective method to evaluate the value of MSC transplantationin patients with ischemic myocardium.
