摘  要:目的研究缺血后处理对于肺缺血一再灌注损伤的保护作用。方法60只雄性sD大鼠随机分为2个模型制作组,每个模型制作组30只随机分为5组:假手术组(S)、缺血一再灌注(I/R)45min组和120min组,以及缺血后处理(IPO)45min组和120min组。用免疫组化法测定肺组织中肺表面活性蛋白A(sP—A)的表达水平,动脉血行动脉血气分析,测定肺组织的通透性(^125Ⅰ标记牛血清白蛋白的漏出量表示),光镜及电镜下观察肺组织的病理形态变化。结果IPO组与I/R组相比较,SP—A表达水平明显升高(P〈O.01),血气分析结果明显改善,肺组织的通透性明显降低(尸〈0.01),肺组织损伤减轻。结论缺血后处理可以减轻肺缺血一再灌注损伤,主要表现为肺组织氧合的改善,尤其是减轻组织水肿和减少Ⅱ型肺泡上皮细胞及肺泡表面活性物质的破坏。Objective To investigate the effect of ischemia post-conditioning on the rat lung during the ischemia-reperfusion injury by measuring the expression levels of SP-A of the lung tissue and the variation of ar- terial blood gas. The permeability and the pathological changes of the lung tissue were observed. Methods Sixty male Sprague-Dawley rats were randomized into two model built groups with 30 animals in each group, then each model built group were randomized into five groups with 6 animals in each group: (1)sham operation group (S). (2) I/R 45 mins group(IR45 ). (3)I/R 120 rains group(IR120). (4)Post-conditioning 45mins group(P45 ), (5)post-con- ditioning 120 mins group (P120). The expression levels of SP-A of lung tissue were detected by immuno-histo- chemical s!ain. Arterial blood gas analysis was conducted. The permeability of lung tissue was determinated (indi- cating by the leakage of ^125Ⅰ bovine serum albumin). The pathological changes of the lung tissue were observed un- der light microscopy and electron microscopy. Results In IPO group, the level of SP-A in lung tissue increased markedly compared with I/R group. The permeability of lung tissue decreased significantly, and the pathologic le- sion of lung tissues were slight. Conclusion Post-conditioning could reduce lung I/R injury, the lung protection of IPO show by improvement of the oxygenation, relieve of lung tissue edema and inhibition of the destruction of alveolar epithelial cell type Ⅱ and alveolar surfactant especially SP-A.
