米氮平与西酞普兰治疗伴有焦虑症状抑郁症的对照研究

摘　　要：目的评价米氮平与西酞普兰治疗伴有焦虑症状抑郁症的疗效和安全性。方法将符合ICD-10中抑郁症及广泛焦虑障碍诊断标准的门诊和住院患者共50例,随机分为两组,分别给予米氮平(研究组)和西酞普兰(对照组)治疗。用汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)和副反应量表(TESS)评定疗效和副反应。结果①米氮平组治愈7例,显进9例,进步4例,无效5例;西酞普兰组分别为4、8、7、6例,两组差异无统计学意义;②显效时间米氮平组3～8天、西酞普兰组11～20天,两组差异有统计学意义(t=-3.86,P<0.01),米氮平起效较快;③焦虑症状的疗效,米氮平组优于西酞普兰组(t=5.39,P<0.05);④副反应方面两组无差异。结论米氮平与西酞普兰对抑郁症疗效接近,前者起效较快,副反应小,且能明显改善焦虑症状及睡眠。Objective To evaluate the efficacy and safety of mirtazapine and citalopram in treating depression associated with anxiety symptoms. Methods A total of 50 outpatients and inpatients who met the international classification of diseases--10（ICD --10）diagnostic criteria for depression and generalized anxiety disorder were randomly divided into two groups,mirtazapine（study group）and citalopram（control group）were given respectively. Efficacy and side effects were assessed by Hamilton Depression Scale （HAMD） ,Hamilton anxiety scale（HAMA） and Treatment Emergent Symptom Scale（TESS）. Results （~）7 cases were successful- ly treated, 9 significantly advanced,4 improved ,and 5 ineffective in mirtazapine group ;while in citalopram group,the cases were 4, 8,7,6 respectively ,differences between two groups were not statistically significant ;（2）The effectual time of mirtazapine group was 3-8 days,while that of citalopram group was 11 - 20 days ,differences between two groups were statistically significant （t=-3.86, P〈0. 01）,mirtazapine worked faster （3） Mirtazapine group was better than citalopram group in the efficacy of anxiety symptoms; （4）No differences were found in side effects. Conclusion The efficacy of mirtazapine and citalopram come close to each other,the former works faster with few side effects,and can improve anxiety symptoms and sleep apparently.
