川芎嗪对胎儿生长受限孕妇脐血流变化的影响研究

摘　　要：目的:探讨川芎嗪用于治疗胎儿宫内生长受限(FGR)孕妇脐血流的影响。方法:选择60例FGR孕妇随机分为川芎嗪组30例(A组)和营养支持组30例(B组),另选择同期进行定期产检并分娩的正常孕妇50例作为正常妊娠对照组(C组)。A组行川芎嗪注射液治疗加营养支持,B组给予单纯营养支持治疗,C组嘱左侧卧位不给予其他处理。监测3组孕妇的脐动脉血流、血液流变学参数及新生儿发育情况及体重。结果:经过治疗后,A组的BPD、HC、AC、FL、EFW增长明显,与B组相比差异有统计学意义(P<0.05);A组孕妇脐血流阻力及血液流变学参数明显低于B组,差异有统计学意义(P<0.05);A组的治愈率及总有效率比B组更高,差异有统计学意义(P<0.05)。结论:川芎嗪能明显改善FGR孕妇血液流变性,降低子宫胎盘的循环阻力,改善胎盘绒毛微循环,促进胎儿宫内生长发育,增加新生儿出生体重,总有效率高。Objective： To explore the impaction of tetramethylpyrazine on hemorheology of pregnant woman with fetal growth re- striction （FGR） Methods ： 60 cases of pregnant women with FGR were randomly divided into tetramethylpyrazine group （ group A） and nu- tritional support group （ group B） , each group had 30 cases. Besides, 50 normal pregnant women who taken regular inspection at the corre- sponding time were selected into the control group （group C） . Patients in group A were treated with tetramethylpyrazine combined with nu- tritional support, patients in group B were only given the nutritional support therapy, and patients in group C were suggested to keep left - lateral position without any other treatment. Parameters such as maternal hemorheology, umbilical artery blood flow velocity, newborn birth weight were measured and compared among the three groups. Results： After treatment, the uterus＇BPD, HC, AC, FL, EFW in group A were increased obviously, and the differences between group A and group B were significant （ P 〈 0. 05 ） . The value of hemorheology pa- rameters and umbilical artery blood flow resistance indexes of group A were significantly reduced than those of group B, the differences were statistically significant （P 〈 0. 05） . Conclusion： Tetramethylpyrazine can obviously improve the hemorheology of FGR patients, reduce uteroplacental resistance and take a favorable turn of placental villus microcirculation, which also can promote fetal growth and development, increase newborn birth weight.
