生物瓣衰坏后再次换瓣的体会(附17例报告)

摘　　要：总结 17例生物瓣衰坏患者的临床表现、再手术方法及术后处理。本组 17例患者中 ,男 7例 ,女 10例。生物瓣植入时 ,单纯二尖瓣置换 15例 ,单纯主动脉瓣置换 2例 ,均为牛心包瓣 ,植入体内时间平均 9.8± 1.4年。术中发现 17枚生物瓣均有不同程度的纤维化或钙化 ,拆除后全部置换机械瓣。术后早期无死亡 ,晚期死亡 2例。生物瓣衰坏随植入时间延长逐渐加重 ,心肌保护采用持续温血停搏液灌注效果好 ,预充抑肽酶能明显减少术后出血 。The experience of reoperation in patients due to degeneration of bioprosthetic valves was reviewed in this report 17 patients(7men,10women)underwent re replacement of previously implanted bioprosthesis for it’s structural failure in the period from 1992 until 2000 Retrospective analysis revealed a mean implantation time of (9 8±1 4) years for the bioprostheses At reoperation 15 of them had a mitral valve replacement,2 had a aortic valve replacement There were no perioperative death Late death was found in 2 patients The dysfunction of bioprosthesis is most often caused by leaflet calcification and thickening With continuous warm blood cardioplegia,aprotinin and postoperative support,good results can be made in patients with low functional status
