摘 要:目的探讨口腔修复科交叉感染的危险因素,总结预防对策,为控制医院感染提供依据。方法回顾性分析医院口腔修复科2010年1月-2011年12月的120例患者,对其进行乙型肝炎病毒检测,比较乙型肝炎病毒感染组和未感染组治疗过程中的空气质量、器械消毒灭菌、废物处理和易感人群情况。结果 120例患者HBsAg阳性患者有42例,阳性率为35.0%;未感染组治疗过程中诊室空气中细菌平均数量为(2.3±0.2)CFU/m3,感染组(3.5±0.3)CFU/m3;器械表面的细菌数量,感染组(102±11)CFU/cm2,未感染组(62±13)CFU/cm2;感染组易感人员24例,未感染组25例,两组间比较差异有统计学意义(P<0.05);诊疗过程中未感染组的废物处理要优于感染组。结论空气质量、器械消毒灭菌、垃圾处理和易感人群等因素均与口腔修复科交叉感染密切相关,必须严格执行口腔修复科技术操作规程,健全管理制度,确保口腔器械消毒、灭菌质量,加强医疗废物管理,才能降低口腔修复科交叉感染发生率。OBJECTIVE To explore the risk factors of cross infections in department of prosthodontics and to put forward the prevention measures so as to provide basis for the control of nosocomial infections. METHODS A total of 120 cases of patients, who were treated in the department of prosthodontics from Jan, 2010 and Dec, 2011, were selected and retrospectively analyzed. The patients were tested for hepatitis B virus ,and the related factors were compared between the infection group and the non-infection group in their treatment process, such as air quality, equipment disinfection, waste treatment and population susceptible to infection. RESULTS Of totally 120 patients, there were 42 cases with HBsAg tested positive, the positive rate was 35.0%. The average bacterial colony counts in the air of clinic during the treatment was(2.3±0.2) CFU/m3 of the non-infection group and (3.5 ±0.3) CFU/m3 of the infection group; "the bacterial colony counts on the instrument surface was (102 ±11) CFU/cm2 of the infection group and (62± 13)CFU/cm2 of the infection group; there were 24 cases in the infection group and 25 cases in the non-infection group who were prone to be infected, the difference between the two groups was statistically significantl the disposal of medical waste of the non-infection group was better than that of the infection group. CONCLUSION The incidence of cross infections in the department of prosthodontics is closely related to the factors such as the air quality, equipment disinfection, waste treatment, and the population susceptible to infections. Only when the technical operation procedures are strictly implemented in the department of prosthodontics, the management system to be made sound, the disinfection of oral equipment and the sterilization quality to be ensured, and the management of medical waste to be intensified can redule the incidence of cross infections in the department of prosthodontics.
