全关节置换术前后白细胞介素-6和C反应蛋白变化及临床意义

摘　　要：目的研究血清白细胞介素-6(IL-6)和C反应蛋白(CRP)在全关节置换手术前后变化及其临床意义。方法前瞻性研究2012年1～12月期间61例初次人工节置换患者,其中全髋关节置换30例,全膝关节置换31例,于术前及术后1、3、5、7、12 d检测并记录IL-6和CRP。结果手术后IL-6和CRP测定值均增高,IL-6在手术后1 d达到峰值(27.79±20.83 pg/ml),以后逐渐下降,术后12 d降至正常;CRP在术后3 d达到峰值(73.57±44.37 mg/L),术后12 d仍明显高于正常。结论全关节置换术后血清IL-6值变化比CRP快,回落到正常的速度也快。IL-6值能较早、较快反映全关节置换术后炎症情况,动态观察有助于早期感染的发现和干预。Objective To study the serum levels of interleukin-6 （IL-6） and C-reactive protein （CRP） before and after total joint replacement and its clinical significance. Methods A prospective study of 61 patients with primary joint replacement, including 30 cases of total hip replacement and 31 cases of total knee replacement during the period from January 2012 to December 2012, was designed to observe the changes of IL-6 and CRP leval before replacement and in the first, third, fifth, seventh, twelfth day after replacement. Results The value of IL-6 and CRP in patients were increased after replacement. The serum IL-6 leval increased rapidly, peaked on the first postoperative day at maximum levels （27. 79 ± 20.83 pg/rnl）, and dropped gradually to preoperative leval at the twelfth day. In comparison, the postoperative CRP leval reached maximum levels （73.57 ± 44. 37 mg/L） at the third day, and were also apparently higher than preoperative leval at the twelfth day. Conclusions Serum IL-6 levels changed more rapidly than CRP levels after total joint replacement, and returned to normal more rapidly. IL-6 can early, rapidly reflect the status of inflammation, and dynamic observation of IL-6 can contribute to an early diagnosis and intervention of infection.
