胰岛素样生长因子结合蛋白-1用于胎膜早破的快速诊断

摘　　要：目的探讨胰岛素样生长因子结合蛋白一1在胎膜早破中的快速诊断价值。方法选取38例胎膜早破病例为胎膜早破组，82例胎膜未破病例为对照组，对其宫颈分泌物分别予以国内、外胰岛素样生长因子结合蛋白一1测试纸条检测，并结合石蕊pH试纸进行分析，比较三者间的灵敏度、特异度、阳性预测值、阴性预测值。结果胰岛素样生长因子结合蛋白一1试验产品用于快速诊断胎膜早破在灵敏度、特异性、阳性预测值、阴性预测值与国外对照产品有很高的一致性X2值分别为5．591、1．580、5．583、6．790，除特异度P〉0．05外，余均P〈0．05。结论国内、外两种胰岛素样生长因子结合蛋白一1测试纸条监测胎膜早破的一致性很高，与石蕊试纸比较在检测胎膜早破中均有明显的优越性。Objective To explore the value of insulin-like growth factor binding protein-1 （ IGFBP-1 ） in the rapid diagnosis of premature rupture of membrane （PROM）. Methods Thirty-eight cases of PROM were selected in PROM group, and 82 cases with unruptured membrane were in control group. Cervical secretions of two groups were detected by domestic and abroad IGFBP-1 test strip. Litmus pH test paper was also used for analysis. The sensitivity, specificity, positive predictive value and negative predictive value of three products were compared. Results IGFBP-1 test product has very high consistency with abroad control product in sensitivity, specificity,positive predictive value and negative predictive value for rapid diagnosis of PROM （X2 value was 5. 591, 1. 580, 5. 583 and 6. 790, respectively）. Except P value was more than 0.05 in specificity, the other P value was less than 0.05. Conclusion Domestic and abroad IGFBP-1 test strip are highly consistent in monitoring PROM. Compared with litmus test paper, they have obvious advantages in the detection of PROM.
