会阴部湿疹样癌切除术前多点活检增加手术精准性的应用探讨

摘　　要：目的总结会阴部湿疹样癌切除术前多点活检增加手术精准性的临床经验。方法对已确诊的患者在术前1-2d，分别在距肿瘤边界1、2、3cm外缘的1、3、5、7、9、11点的位置取材送冰冻病理诊断，共18个点。根据冰冻病理结果，确定肿瘤的切除范围。结果5例患者中，术中首次切缘冰冻均为阴性，无须反复送检。结论术前多点活检，提高了会阴部湿疹样癌扩大切除术的精准性，避免因过大切除而增加对创面修复的困难，同时也减少了因肿瘤切除不彻底而反复送检冰冻的次数，节省了手术时间。Objective To summarize the clinical experience of application of preoperative multiple punch biopsy in increased accuracy of perineal paget disease resection. Methods Within preoperative 1 or 2 days, bi- opsy samples at 18 points from each patient were harvested at the 1,3, 5, 7, 9, 11 clock point which were 1,2, 3 cm away from outer margin of tumor respectively. The scopes of tumor resection were determined based on the results of frozen pathology. Results Among 5 patients, The first results of samples from incisional margin were negative by fast-frozen pathology. There was no need inspected repeatedly. Conclusion Preoperative multiple punch biopsy could enhance the of excising accuracy of perineal Paget disease, avoiding large defect owing to ex- cessive resection which was hard to repair and decrease the frequency of frozen pathologies because of overly con- servative resection. This method can significantly save the operative time.
