斑点追踪成像评价原发性高血压病人左心室扭转及解旋功能

摘　　要：目的应用斑点追踪成像检测原发性高血压病人左心室扭转及解旋运动的变化,探讨其临床意义。方法选取原发性高血压病人61例,其中室间隔肥厚组28例,非室间隔肥厚组33例,另选健康志愿者30例为正常对照组。应用斑点追踪成像计算左心室峰值扭转角度(Ptw)、收缩期末扭转角度(AVCtw)、等容舒张期末扭转角度(MVOtw)和解旋率(UntwR)。结果与正常对照组相比,Ptw、AVCtw、MVOtw在室间隔肥厚组及非室间隔肥厚组均增大(F=14.86～16.70,q=5.69～32.05,P<0.05);与正常对照组及非室间隔肥厚组相比,室间隔肥厚组的UntwR减小(F=18.55,q=6.53、18.90,P<0.05)。结论原发性高血压病人左心室扭转运动增大,解扭转运动在高血压室间隔肥厚病人减低,斑点追踪成像为高血压病人心肌运动的评价提供了一个新途径。Objective To investigate left ventricular twisting and untwisting in primary hypertension patients and its clinical significance by using speckle tracking imaging(STI).Methods Sixty-one patients with primary hypertension-including 28 with concurrent septal hypertrophy and 33 non-septal hypertrophy-were enrolled,and 30 healthy volunteers selected to serve as controls.By employing STI,peak left ventricular torsion(Ptw),twist at aortic valve closure(AVCtw),twist at mitral valve opening(MVOtw) and untwisting rate(UntwR) were calculated.Results Ptw,AVCtw and MVOtw enlarged in both septal hypertrophy and non-hypertrophy groups versus normal controls(F=14.86-16.70,q=5.69-32.05,P<0.05),compared with both the normal control and non-septal hypertrophy,the UntwR in septal hypertrophy group diminished(F=18.55;q=6.53,18.90;P<0.05).Conclusion Twisting motion of left ventricle enhances in patients with primary hypertension,and untwisting declines in those with concurrent septal hypertrophy.STI provides a new way to assess the myocardial motion in hypertensive patients.
