吸入性糖皮质激素对哮喘病儿血清25-(OH)D3和总IgE影响

摘　　要：目的探讨血清25-羟维生素D3(25-(OH)D3)和总IgE的水平与支气管哮喘发病的关系,以及长期吸入糖皮质激素对两者影响。方法检测65例哮喘病儿用药前及吸入糖皮质激素治疗3、6、12个月时血清中25-(OH)D3和总IgE的水平,并进行比较。结果随吸入糖皮质激素治疗时间的延长,血清25-(OH)D3水平呈上升趋势,总IgE水平呈下降趋势,差异均有统计学意义(H=95.296、23.037,P均<0.01),且两者之间呈负相关关系(r=-0.614,P<0.01)。结论维生素D缺乏可能是导致支气管哮喘发病的重要原因之一,且血清25-(OH)D3水平增高可能抑制IgE的过度表达。长期吸入糖皮质激素对病儿骨代谢25-(OH)D3途径无明显影响。Objective To discuss the relationship between the levels of serum 25-hydroxyvitamin D3(25-(OH)D3 and total IgE as well as the pathogenesis of asthma,and the effects of long-term treatment with inhaled corticosteroids on both 25-(OH)D3) and total IgE.Methods The levels of serum 25-(OH)D3 and total IgE were detected in 65 children with asthma-before and after treatment with inhaled corticosteroids for 3,6 and 12 months.Results The levels of serum 25-(OH)D3 elevated along with extension of time of therapy with inhaled corticosteroids,and levels of total IgE decreased(H=95.296,23.037;P<0.01),the levels between 25-(OH)D3 and IgE were negatively correlated(r=-0.614,P<0.01).Conclusion Vitamin D deficiency may be one of the important factors contributing asthma,and the increase of serum 25-(OH) D3 is likely to inhibit the overexpression of IgE.Long-term inhaled corticosteroids therapy has no significant effects on 25-(OH)D3 pathway of bone metabolism in asthmatic children.
