摘  要:类风湿关节炎(RA)是以关节滑膜炎症为主要病理表现的慢性全身性自身免疫性疾病,以慢性、对称性、多关节滑膜炎和关节外病变为主要临床表现,好发于手、腕、足等小关节,反复发作,呈对称分布,其滑膜炎持久反复发作,可导致关节内软骨和骨的破坏,关节功能障碍,甚至致残。RA发病机制至今尚未阐明。滑膜细胞增生,血管翳形成,骨与软骨破坏是其主要的病理特点。滑膜细胞信号转导异常、滑膜细胞增殖及凋亡失衡导致滑膜细胞增生,基因突变、原癌基因活化及细胞因子等与滑膜细胞的凋亡相关。Rheumatoid arthritis(RA) is an autoimmune disease, the main pathological manifestations of which is synovitis, and the pathogenesis is uncertain. The main clinical manifestations of RA are chronic, symmetrical, multi-joints synovitis and the extra-articular symptoms. The inflammation of synovium are often seen in small joints, such as wrists, hands and feet, which is recurrent and distributes symmetrically, and can subsequently lead to destructions of cartilage and bone, deformity of joints, or even disability. The pathological characteristics of RA are synovial cell proliferation, pannus formation and bone and cartilage destructions. Synovial cell proliferation is caused by abnormal signal transduction pathways and the imbalance of cell proliferation and apoptosis,and the apoptosis of synovial cell is related to gene mutation, proto-oncigene activation and cvtokines.
