心绞痛患者血浆白细胞介素-6和肿瘤坏死因子-α的变化及临床意义

摘　　要：目的研究心绞痛患者白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)的变化趋势及临床意义。方法纳入冠状动脉造影确诊的心绞痛患者70例,包括稳定型心绞痛(SAP)组34例,不稳定型心绞痛(UAP)组36例,另外纳入30例虽有不典型心绞痛症状但冠状动脉造影正常者作为对照组。采用酶联免疫吸附法检测各组血浆IL-6、TNF-α水平。结果对照组、SAP组和UAP组血浆IL-6和TNF-α水平均呈逐渐增加的趋势,IL-6的表达水平分别为:(11.64±2.49)pg/L、(14.79±2.53)pg/L、(18.87±2.58)pg/L;TNF-α的表达水平分别为:(16.08±4.23)ng/L、(19.87±4.43)ng/L和(22.91±4.51)ng/L,组间差异均有统计学意义(P均<0.01)。结论血浆IL-6与TNF-α在反映冠状动脉斑块稳定性及冠状动脉病变的严重程度方面有重要的参考意义。Objective To study the variation trend and clinical significance of plasma interleukin-6 （ IL-6 ） and tumor necrosis factor- α （ TNF- α） in patients with angina peetoris （ AP ） in coronary heart disease （ CHD ） . Methods The AP patients （ n=70 ） diagnosed by coronary angiography （ CAG ） were chosen, among them 34 with stable AP （ SAP group ） , and 36 with unstable AP （ UAP group ） . Other patients （ n-30 ） with atypical AP symptoms but normal CAG were selected as control group. The levels of plasma IL-6 and TNF- α were detected by using enzyme-linked immunosorbent assay （ ELISA ） . Results The levels of plasma IL-6 and TNF- α showed increasing trend in control group, SAP group and UAP group. The expression level of IL-6 was, respectively, （ 11.64 ± 2.49 ） pg/L, （ 14.79 ± 2.53 ） pg/L and （ 18.87 ± 2.58 ） pg/L in three groups, and the expression level of TNF- α was, respectively, （ 16.08 ± 4.23 ） ng/L, （ 19.87 ± 4.43 ） ng/L and （ 22.91 ± 4.51 ） ng/L （ P〈0.01 ） . Conclusion The levels of plasma IL-6 and TNF- α have important reference significance in reflecting the stability of atherosclerotic plaque and severity of CHD.
