托特罗定联合塞来昔布预防性治疗经尿道前列腺电切术后膀胱过度活动症

摘　　要：目的:探讨酒石酸托特罗定联合塞来昔布预防性治疗经尿道前列腺电切术后膀胱过度活动症(overactive bladder,OAB)的疗效。方法:将150例良性前列腺增生症患者随机分为2组:联合用药组、托特罗定组,每组75例。比较2组经尿道前列腺电切术(transurethral resection of prostate,TURP)术后72 h内膀胱区阵发性痉挛性疼痛伴(或不伴)急迫性尿失禁的次数及膀胱痉挛持续时间。结果:联合用药组术后第2、第3天膀胱痉挛次数分别为(1.26±0.92)次、(0.73±0.69)次,膀胱痉挛平均持续时间分别为(0.13±0.13)h、(0.12±0.08)h,均少于托特罗定组[(1.90±0.68)次、(1.12±0.83)次;(0.22±0.14)h、(0.29±0.11)h],差异均有统计学意义(P<0.05)。术后联合用药组的膀胱冲洗时间为(2.2±0.7)d,冲洗液转清时间为(1.7±0.7)d,留置导尿管时间为(5.0±0.8)d,均少于托特罗定组[(2.7±0.8)d、(2.3±0.7)d和(5.4±0.7)d],差异有统计学意义(P<0.05)。结论:TURP术后早期联合应用酒石酸托特罗定片与塞来昔布是预防患者膀胱痉挛的安全、有效的方法。Objective：To evaluate the effect of tolterodine tartrate combining celecoxib suppository for prophylactic treatment of overactive bladder after transurethral resection of prostate （TURP）. Methods： A total of 150 patients with BPH were treated with TURP. All patients were randomly divided into combined drug group and tolterodine group. The number and duration of paroxysmal spasms with（or without）urge incontinence within 72 h after TURP were compared. Results： At 24-48 h and 48-72 h after operation,the frequencies of bladder spasm in combined drug group were （1.26 ± 0.92） and （0.73 ± 0.69） ,less than those of tolterodine group [（1.90 ± 0.68） and （1.12 ± 0.83）] ,with statistically significant differences （P〈0.05）. The duration of bladder spasm in combined drug group were （0.13 ± 0.13）h and （0.12 ± 0.08）h,significantly less （P〈0.05） than those of tolterodine group [（0.22 ± 0. 14）h and （0.29 ± 0.11）h]. The time of bladder irrigation,time needed for washing solution to turn clear and time of catheter indwelling in combined drug group were（2.2 ± 0.7）d, （1.7 ± 0.7）d and （5.0 ± 0.8）d,statisti cally shorter （P〈0.05） than those of tolterodine group [（2. 7 ± 0. 8） d, （2. 3 ± 0. 7）d and （5. 4 ± 0. 7）d]. Conclusions： Tolterodine tartrate tablets with celecoxib suppository are safe and effective for prophylactic treatment of patients with bladder spasm after TURP.
