摘  要:目的研究甲状腺结节内部微小强回声灶的超声特征,并探讨其临床意义。方法前瞻性分析859个结节内部强回声灶的大小、形态、部位、有无多次反射伪像(彗星尾征)及彩色闪烁伪像等超声特征,对比组织病理学结果,判断.上述特征的临床意义。结果强回声灶的大小在良恶性两组的差异无统计学意义。圆形强回声灶在恶性组更多见(90.3%vs 54.0%,P<0.05),恶性组的38例扁长形强回声灶全部位于实质部,良性组的扁长形强回声灶全部位于非实质部。恶性组没有只存在于非实质部的强回声灶,而良性组中88.9%的强回声灶只存在于非实质部(P<0.05)。彗星尾征在两组表现有差异,58个恶性组的强回声灶中有15个(25.9%)彗星尾征位于实质部。结论扁长形且只在非实质部出现的强回声灶是良性征象,彗星尾征及彩色闪烁伪像不能作为诊断良性肿瘤的可靠依据。Objective To analyze the characteristics of echogenic foci in the thyroid nodules, to discuss its clinical significance. Methods A total 859 thyroid nodules were included in this prospective study. The characteristics of echogenic foci, such as size, shape, position, with reverberations(comet tail artifacts)or not, with color flash artifacts or not, were evaluated. Results There was no significant difference in size between two groups. Round echogenic foei was more frequent in malignance. Linear echogenic foci in 38 malignances were all located in solid portion. Linear echogenic foci in 214 benign nodules were all located in non-solid portion. There was no echogenic foci exclusively loca- ted in non-solid portion in malignance. There was significant difference in comet tail artifacts between two groups. 15 echogenic foci with comet tail artifacts were located in solid portion in malignance. Conclusions Echogenic loci with linear shape exclusively located in non-solid portion is a sign of benign nodule. Comet tail artifacts and color flash arti- facts cannot be used as a reliable diagnostic evidence for benign nodule.
