摘 要:目的通过建立体-肺循环分流致慢性肺动脉高压猪模型,应用超声心动图探讨肺动脉瓣血流动力学动态变化情况。方法实验组30只小猪,外科手术建立体肺循环分流模型,假手术组6只。术前及术后每间隔2个月做1次超声心动图,连续监测7次。取肺动脉瓣口血流频谱,测量肺动脉加速时间(PAAT),肺动脉减速时间(PADT),右室射血时间(RVET),计算PAAT/RVET比值。测量三尖瓣反流速度(V)和压差(△P),估测肺动脉收缩压(PASP)。统计学方法用重复测量数据的方差分析。结果 PASIP逐渐增高(P<0.05);不同观测时点的PAAT、PADT、RVET、PAAT/RVET值比较有统计学差异,呈先增高后降低表现(P<0.05)。结论建立左向右分流致慢性肺动脉高压动物模型,随着P.ASP的升高,PAAT、PADT、RVET及PAAT/RVET值呈先增高后降低表现。Objective To investigate the changes of pulmonary valve hemodynamics in a swine model with chron- ic Pulmonary hypertension induced by left to right shunting using Transthoracic Doppler-echocardiography (TDE). Methods 30 experimental swines assigned to experimental group, which were established left to right shunting model by surgical operation,while 6 swines were assigned to false surgical group. In the preoperative and postoperative inter- vals of 2 months, obtained the continuous monitoring of echocardiography seven times. The pulmonary artery spec- trum was acquired to measured pulmonary artery acceleration time (PAAT), pulmonary artery deceleration time (PADT), right ventricular ejection time(RVET). PAAT/RVET was eaIculated. The tricuspid regurgitation spectrum was acquired to measured return flow velocity(V) and pressure(~P). Pulmonary artery systolic pressure(PASP) was estimated. The data were analyzed by repeated measurement data analysis of variance. Results In experimental group,PASP increased gradually(P〈 0. 05)in the process of establishing chronic pulmonary hypertension model, and PAAT,PADT,RVET,PAAT/RVET increased at first and then reduced(P^0.05). Conclusions In the process of establishing animal model with chronic pulmonary hypertension by left to right shunting, with the increasing of PASP, PAAT,PADT,RVET and PAAT/RVET increased at first and then reduced.
