摘  要:目的探讨微泡增强的超声空化效应联合人凝血酶原复合物阻断正常犬前列腺血流灌注的可行性及其机制。方法 6只健康成年雄性犬开腹暴露前列腺后,经静脉滴注20 IU/kg的人凝血酶原复合物,药物滴注完成后立即经静脉持续缓慢推注微泡0.1 ml/kg的同时,予以脉冲式超声空化治疗仪探头直接接触辐照前列腺10 min,靶区实验前后分别行超声造影检查,并分析造影时间-强度曲线,记录峰值强度、达峰时间、曲线下面积,实验后获取前列腺组织行大体和组织病理学检查。结果超声空化联合凝血酶原作用于前列腺后,造影剂的峰值强度由(36.02±6.61)%下降至(24.35±3.15)%,差异有统计学意义(P<0.05);病理学检查可见前列腺内小血管崩解破裂,血栓、血肿形成,间质和部分腺腔内有大量漏出的红细胞。结论微泡增强的超声空化联合凝血酶原对犬前列腺的血流灌注能产生一定的阻断作用,可能机制是前列腺内血管损伤破坏,血栓、血肿形成。Objective To explore the feasibility and the mechanism of blood perfusion obstruction of normal ca nine prostate by microbubbles mediated ultrasound cavitation and pruthrombin. Methods After abdominal opening to expose the prostates, the canine received an intravenous drip of prothrombin (20 IU/kg). Then microbubbles en- hanced ultrasound was performed on prostates for 10 minutes. Contrast-enhanced ultrasonography was performed both before and after treatment, and a time-intensity curve was analyzed to obtain peak intensity, time to peak and area un- der curve. Prostates were removed for gloss observation and histological examination after treatment. Results The peak intensity dropped from (36.02±6.61)% to (24.35± 3.15)% (P〈0.05). Pathological examination were mi crovessle rupture, hemorrhage and thrombus in the prostate. Conclusions Microbubbles mediated ultrasound cavitati- on and prothrombin can partial block blood perfusion of canine prostate. The mechanism could be microvessle rupture and thrombosis in prostate.
