胆肠吻合术后营养支持的护理体会

摘　　要：目的总结胆肠吻合术后营养支持的护理经验。方法回顾性分析90例胆肠吻合术后患者实施营养支持的护理资料。营养支持包括:保护血管;血糖、电解质的监测;空肠造瘘管或鼻空肠营养管的护理;严格掌握肠内营养液滴注的速度、浓度及剂量;口服饮食的指导。结果 90例患者在营养支持治疗期间,8例发生代谢性并发症,5例为高血糖反应,3例为低血糖反应;2例出现肝功能异常;10例出现胃肠道反应。经及时处理及精心护理,并发症很快得到控制,全部病人的营养状况都得到改善并康复出院。结论胆肠吻合术后患者做好营养支持的护理工作,对预防并发症的发生有积极作用。Objective To sum up the nursing experience of nutritional support after biliary-enteric anastomosis. Method Nursing data of 90 cases of patients receiving nutritional support after biliary-enteric anastomosis were analyzed retrospectively. Nutritional support consisted of protecting blood vessel, monitoring blood glu- cose and electrolyte, nursing jejuno-stoma tube or nasointestinal feeding tube; controlling the velocity, concen- tration and dose of enteral nutrition liquid instillation in a strict manner, and providing dietary guidance. Re- suits In receiving the nutritional support, among all the 90 patients, there were 8 cases with metabolic com- plications, 5 cases with hyperglycemia reaction, 3 cases with hypoglycemia reaction, 2 cases with abnormal liv- er function and 10 cases with gastrointestinal reaction. The complications were quickly brought under control after timely treatment and careful nursing. The nutritional status of all patients were improved and they recov- ered and were discharged from hospital. Conclusion Providing well nutritional support for patients after bili- ary-enteric anastomosis plays a positive role in guarding against complications.
