摘　　要：目的探讨综合性护理干预对胶囊内镜在小肠疾病检查中的影响。方法将2011年9月至2012年3月行胶囊内镜检查的61例病例作为对照组,2012年4—10月行胶囊内镜检查的66例病例作为干预组。对照组给予常规护理如查看和询问病史以掌握适应症与禁忌症,说明检查的原因、目的、方法、时间、注意事项、可能引起的并发症及常规的肠道准备方法等。干预组在常规护理的基础上给予综合性护理干预,如充分术前评估、个性化心理护理、严密监督肠道准备过程、加强胶囊运行速度的实时监控,对胶囊在胃内滞留时间过长或胶囊运行速度过快的患者进行体位变换等指导,确保胶囊在有效时间内完成全小肠检查。观察两组患者肠道清洁度、胶囊内镜检查成功率及病变检出率的差异。结果两组胶囊内镜检查患者的肠道清洁度、检查成功率及病变检出率的比较,差异有统计学意义,P均小于0.05。结论综合性护理干预对胶囊内镜在小肠疾病检查中的成功和病变检出等方面起着重要的作用。Objective To Investigating the effect of comprehensive care on capsule endoscopy in examination small intestine disease. Methods We took 61 patients who did capsule endoscopy during February 2011-October 2011 as the control group, then other 66 cases taken capsule endoscopy during November 2011- August 2012 as the intervened group. The control group was only given routine care, such as to inform patients the working principle of capsule endoscopy, the method of examination and taking laxatives. And the intervention group was additional given comprehensive nursing intervention on the basis of conventional care, such as personalized endoscopy knowledge education, sufficient preoperative assessment, strict indications and contraindications, close supervision of the intestine preparation process, strengthening the real-time monitoring on the operating speed of the capsule, and guiding patients who~ the residence time in the stomach too long or the capsule run too fast to change postural, then ensured they complete the examination within the effective time. The two groups were observed in intestinal cleanliness, the success rate of capsule endoscopy examination and the detection rate of lesion. Results The comparison of the intestinal cleanliness, examination success rate and lesion detection rate of the above two groups, that the diversity was statistically significant, P 〈 0. 05. Conclusions Comprehensive care intervention plays an important role for capsule endoscopy in success examination in small intestine diseases and lesion detection.
