探讨糖尿病肾病患者肾功能状况与甲状腺功能的关系

摘　　要：目的分析糖尿病肾病患者的肾功能状况与甲状腺功能的关系,为临床治治疗提供有价值的参考依据.方法抽取在我院进行诊治的糖尿病肾病患者50例、糖尿病早期患者50例及健康查体者50例,分别对其游离T3（FT3）、游离T4（FT4）、促甲状腺激素（TSH）水平采取检测后进行对比分析.结果健康查体组及糖尿病早期患者组的游离T4（FT4）、促甲状腺激素（TSH）水平没有显著差异,部分糖尿病早期患者的游离T3（FT3）水平有所减低,所有糖尿病肾病患者的游离T3（FT3）、游离T4（FT4）水平均呈明显下降趋势而促甲状腺激素（TSH）水平则有升高趋势.结论糖尿病肾病患者的甲状腺激素水平随着其病情的加重有逐渐降低趋势,在临床肾病期患者的甲状腺功能处于亚临床甲状腺功能减退状态,在肾病终末期患者的甲状腺功能则处于低T3综合征状态,因此,通过检测糖尿病肾病患者的甲状腺功能,能够为临床评估其肾功能状况及其预后提供有价值的参考依据.Objective To analyze the relationship of the diabetic nephropathy patients＇s renal function and thyroid function,and to provide a valuable reference for the treatment of clinical governance.Methods 50 patients with diabetic nephropathy and 50 patients with early stage diabetic receiving treatment and 50 healthy people were brought together.All of them were taken the detection of free T3 （FT3）,free T4 （FT4） and thyroid stimulating hormone （TSH）.The results were compared and analyzed.Results There was no significant differences in free T4 （FT4）,thyroid stimulating hormone（TSH） between the health group and the early diabetes group,except the free T3 （FT3） level of part of the early diabetes patients had reduced.Levels of free T3 （FT3） and free T4 （FT4） of all diabetic nephropathy patients showed a clear downward trend but the level of thyroid stimulating hormone（TSH） showed a upward trend. Conclusion：Thyroid hormone levels of patients with diabetic nephropathy gradually decreased with the aggravation of their disease,thyroid function of patient rendered subclinical hypothyroidism state in clinical nephrology period and low T3 syndrome state in nephrotic end-stage.Therefore,we can provide a valuable reference for the clinical assessment of renal function status and prognosis of thyroid function through the detection of patients with diabetic nephropathy.
