部分性重复膀胱误诊为阴道斜隔综合征一例及相关文献复习  

Incomplete Duplicated Bladder Misdiagnosed as Oblique Vaginal Septum Syndrome:A Case Report and Literature Review

摘　　要：目的探讨部分性重复膀胱畸形和阴道斜隔综合征的临床特征及诊断、治疗方法,避免误诊。方法回顾性分析1例误诊为阴道斜隔综合征的部分性重复膀胱的临床资料。结果本例因痛经11年,婚后不孕2年就诊。双合诊示阴道右侧壁囊性感,B超检查示阴道斜隔、右肾缺如,诊断为阴道斜隔综合征。经手术证实为部分性重复膀胱畸形、右侧肾脏发育不良、右侧输尿管积水。术后予抗感染治疗,痊愈出院。结论部分性重复膀胱畸形临床少见,与阴道斜隔综合征同属于泌尿生殖系统先天性畸形,二者临床表现相似,易相互误诊,提示诊断时应详细询问病史,仔细查体,并及时完善相关检查。Objective To explore clinical features,diagnosis,treatment of incomplete duplicated bladder and oblique vaginal septum syndrome,so as to avoid misdiagnosis.Methods Clinical data of one patient with incomplete duplicated bladder misdiagnosed as having oblique vaginal septum syndrome,was retrospectively analyzed.Results The patient visited a doctor for 11 years of dysmenorrhea and 2 years of infertile after getting married.Bimanual examination showed cystic mass on the right side wall of vagina,ultrasound examination showed oblique vaginal septum and right renal agenesis,and oblique vaginal septum syndrome was diagnosed.Incomplete duplicated bladder,the right kidney dysplasia,and right hydroureter were confirmed by surgical exploration.The patient was cured and discharged after postoperative anti-infective therapy.Conclusion Incomplete duplicated bladder is rare in clinical practice.Both incomplete duplicated bladder and oblique vaginal septum syndrome belong to congenital anomaly of urogenital system.The two diseases are easily misdiagnosed because of similar clinical features.Clinical diagnosis should be based on detailed history,careful physical examination and other related examinations.
