高能量型胫骨平台骨折的个体化治疗策略

摘　　要：目的:分析总结高能量所致的复杂胫骨平台骨折的个体化治疗策略。方法:回顾性分析我院收治的SchatzkerⅤ型、Ⅵ型复杂胫骨平台骨折患者资料34例,对不同病例,分别选用螺钉、单侧钢板、双侧钢板内固定治疗,部分使用外固定支架作为临时辅助复位固定工具或者保守治疗。结果:经6个月～5年随访,平均28个月,骨折均愈合,参照Rasmussen评分标准,术后6个月优良率82.4%,术后1年优良率79.4%。结论:手术切开复位牢靠内固定或结合外固定是治疗复杂胫骨平台骨折的有效方法,注意严密的术前检查,对不同类型的高能量所致的复杂胫骨平台骨折采用个体化治疗方法,是提高疗效的关键。Objective： To analyze and summarize caused the highenergy complex tibial plateau fracture surgery recon struction strategy. Methods： A retrospective analysis of our hospital Schatzker V type, the complex tibial plateau fractures were 34 cases of unilateral steel or bilateral steel plate fixation for the treatment of different cases were select ed,other patients with external fixator as an auxiliary tool. Results：The patients after 6 months to 5 years of followup the （mean 28 months）, all patients with fractures healed, the treatment effect reference to the the Rasmussen score： 6 months after excellent rate of 82. 4 after 1 years excellent rate of 79.4. Conclusion：The use the internal fixation or external fixator is an effective method the the treatment the complex tibial plateau fractures. But pay attention to the physical and imaging examinations before the surgery, and individualized treatment is the key the improve the efficacy of complex tibial plateau fractures.
