肺炎支原体感染诱发小儿哮喘36例临床分析

摘　　要：目的:探讨肺炎支原体(Mycoplasma pneumoniae,MP)感染诱发的小儿哮喘的临床特征及治疗效果。方法:对74例哮喘发作患儿的临床资料进行回顾性分析,其中MP感染诱发哮喘36例(MP感染诱发组),非MP感染诱发哮喘38例(非MP感染诱发组)。观察2组患儿的发热、咳嗽、喘息、肺部病变、肺外脏器受累情况,以及治疗措施和对支气管扩张剂的治疗反应等。结果:MP感染诱发组患儿在发热、咳嗽、喘息、肺部病变及对支气管扩张剂的反应等方面与非MP组差异均有统计学意义(P<0.05～P<0.01);2组患儿肺外脏器受累发生率差异无统计学意义(P>0.05)。MP感染诱发组应用大环内酯类抗生素治疗效较好。结论:与非MP感染诱发的小儿哮喘比较,MP感染诱发哮喘发热多见、咳嗽剧烈、喘息出现晚且持续时间长,对支气管扩张剂不敏感,大环类酯类抗生素联合支气管扩张剂和糖皮质激素吸入治疗有效。Objective：To explore the clinical characteristics of children asthma caused by infection of Mycoplasma pneumonia （MP） and treatment effect of such diseases. Methods：The clinical data of 74 cases diagnosed as asthma were analyzed retrospectively from 2009 to 2011. Thirty-six cases were diagnosed as asthma caused by infection of MP（infection of MP group） and 38 cases of asthma caused by non-infection of MP （ non-infection of MP group）. The patients fever, cough, wheezing characteristics, lungs function, organs involved extra pulmonary organs, therapies and the treatment responses to bronchodilators were observed. Results： The two groups showed statistical differences in the measures of fever, wheezing characteristics, lungs function and the treatment responses to bronchodilators（P 〈 0.05 - P 〈 0.01 ）. The incidence of lung involvement was not statistically significant （ P 〉 0.05 ）. Macrolide antibiotics must be used in the children asthma caused by infection of MP. Conclusions ： Compared with non-infection of MP group, infection of MP group has more fever, severe cough with late wheezing for a long time and insensitivity to bronchodilators. The use of macrolide antibiotics along with bronchodilators and inhaled corti-costeroids is effective in the treatment.
