舒洛地特对原发性膜性肾病的疗效观察

摘　　要：目的通过观察舒洛地特与替米沙坦对原发性膜性肾病（IMN）中、低危患者的疗效，评价舒洛地特对减少原发性膜性肾病尿蛋白的有效性。方法将经过肾脏活检征实，并排除糖尿病肾病、系统性红斑狼疮-狼疮肾炎、病毒性肝炎相关性肾病等继发性膜性肾病的51例IMN中、低危患者随机分为3个治疗组：舒洛地特组，替米沙坦组和联合治疗组，所有患者均随访6个月，随访过程中定期监测患者的24h尿蛋白定量、血浆白蛋白、肾功能、肝功能、血脂及不良反应。结果治疗后三组患者尿蛋白水平及血浆白蛋白水平较治疗前均明显改善，其中舒洛地特组24h尿蛋白定量水平较治疗前下降44．36％，替米沙坦组下降48．74％，联合治疗组下降72．24％。舒洛地特组尿蛋白量下降水平与替米沙坦组比较差异无统计学意义（P〉0．05），两组与联合治疗组比较差异均有统计学意义（P〈0．05）。各组患者血肌酐、尿素氮、血脂及肝功能在治疗前后比较差异无统计学意义（P〉0．05）。结论舒洛地特联合替米沙坦能有效减少IMN中、低危患者的尿蛋白水平，效果优于单一用药。Objective To evaluate the effectiveness of sulodexide reducing urine protein of idio- pathic membranous nephropathy （IMN） by comparing the curative effects of sulodexide and telmisartan on the medium and low-risk patients of IMN. Methods Fifty-one patients having been confirmed IMN by renal biopsy, excluding secondary membranous nephropathy generated from DN, SLE-LN, viral hepatitis associated nephropathy, were devided into three treatment groups： sulodexide group, telmisartan group and combination group. To test 24 hours urine protein, plasma albumin, renal function, liver function, blood lipid and untoward effections at regular intervals in the six months＇ following-up visit. Results Compared with those before the study, 24 hours urine protein significantly reduced in the total three groups, respectively was 44.36% , 48.74% , 72.24%. There was no statistical significance be- tween sulodexide group and telmisartan group （ P 〉 0.05 ）. However, compared with combination group, both groups had statistical significances（P 〈 0.05）. Scr, BUN, blood lipid, renal function and liver function did not significantly difference before and after treatment （ P 〉 0.05 ）. Conclu- sions For decreasing urine protein to the medium and low-risk patients of IMN, taking the oral sulo- dexide has the comparatively curative effects with telmisartan. Sulodexide combined with telmisartan can significantly reduce the 24 hours urine protein.
