摘  要:目的观察高压氧(hyperbaric oxygen,HBO)治疗跟骨骨折局部软组织损伤的临床疗效。方法选取跟骨骨折严重并发症患者94例,经患者知情同意,并签署知情同意书后采用数字表法随机分为对照组和HBO组,每组47例。治疗前统计学分析表明,2组年龄均数和基础病情具有可比性(P〉0.05)。对照组给予常规治疗(应用抗生素、扩血管药、利水消肿药、局部红外线照射),HBO组在上述基础上给予HBO治疗。2组患者经20d治疗后评定患处肿胀、疼痛及血运改善情况。结果2组患者治疗后,发现HBO组肿胀、疼痛较对照组改善早,没有出现局部皮肤坏死情况;而对照组症状改善慢,47例患者中有6例在观察周期结束后伤口未愈,2例皮缘发黑。HBO组有效率(97.9%)明显优于对照组(87.2%),差异有统计学意义(P〈0.01)。结论HBO可以改善跟骨局部缺氧状态,增加局部血液供应,减轻肿胀,促进受损组织修复,有效预防跟骨皮缘坏死。Objective To observe clinical therapeutic effects of hyperbaric oxygen (HBO) on calcaneal fracture with local soft tissue injury. Methods With the knowledge and consent of the patients and following the signing of the letter of agreement, 94 cases of calcaneal fracture with serious complications were divided into the control group and the treatment group. Statistical analyses of the patient basic conditions of the 2 groups indicated that their age and basic illness status were all comparable( P 〉 O~ 05 ). The control group was given routine treatment (antibiotics, vasodilator, and discutientia, local infrared radiation ), while the treatment group received HBO therapy in addition to routine treatment. Swelling, pain sensation and blood supply of the affected areas in the patients of the 2 groups were evaluated, after 20 days of treatment. Results Following treatment, swelling and pain improved earlier than those of the control group, with no signs of local skin necrosis. On the other hand ,. symptoms in the patients of the control group improved slowly, wound sites in 6 of the 47 cases failed to heal at the end of the observation period, and the edge of the skin surface was blackish in 2 cases. The effective rate for the HBO group (97.9%) was obviously superior to that of the control group (87.2%) (P 〈 0, 01 ),with statistical significance. Conclusions HBO therapy could improve local hypoxia in the affected calcaneus, increasing blood supply, alleviating swelling, promoting repair of damaged tissues and preventing skin edge necrosis.
