摘　　要：目的探讨保留悬雍垂腭咽成形术(H-UPPP)治疗阻塞性睡眠暂停低通气综合征(OSAHS)的临床疗效。方法根据术前定位诊断结果,30例OSAHS患者行多平面H-UPPP治疗,比较治疗前后呼吸暂停低通气指数(AHI)、最低血氧饱和度(SaO2)、睡眠参数及Epworth嗜睡评分(ESS)。结果术后1年治愈率为30%,显效率为56.7%,有效率为10%,总有效率为96.7%,术后6个月及1年AHI、最低SaO2及ESS嗜睡评分均较术前明显改善(P<0.01)。结论应用H-UPPP手术治疗OSAHS,治疗效果明显,术后咽腔接近正常解剖形态,患者并发症少,易接受,具有临床推广应用价值。Objective To investigate the clinical efficacy reserved uvula uvulopalatopharyngoplasty （ H - UPPP） in the treatment of obstructive sleep apnea hypopnea syndrome （OSAHS）. Methods According to the results of preoperative localization ,30 OSAHS patients with multi - planar H - UPPP treatment before and after treatment apnea hypopnea index （AHI） ,lowest oxygen saturation （SaO2 ）, sleep parameters and Epworth sleepiness scale （ESS）. Results The postop- erative 1 - year cure rate of 30% , effective rate of 56.7% , the effective rate was 10% , with a total effective rate of 96.7% after 6 months and 1 year AHI,minimum SaO2 and ESS the sleepiness score compared with surgery before signifi- cant improvement （ P 〈 0. 01 ）. Conclusions The application of H - UPPP surgery OSAHS treatment has curative effect, postoperative pharyngeal near normal anatomy, patients with fewer complications and therefore easy to accept, worthy of application.
