北京市男男性接触者HIV与肛门高危型HPV感染的关联性研究

摘　　要：目的了解北京市男男性接触者人类免疫缺陷病毒和肛门人乳头状瘤病毒感染的关系,为我国艾滋病性病防治提供依据。方法通过非政府组织招募男男性接触者进行问卷访谈,采集血液和肛门拭子标本后分别进行HIV-1血清学检测和人乳头瘤病毒DNA基因分型。采用χ2检验、秩和检验、秩相关等方法进行统计分析。结果共调查北京市男男性接触者289人,HIV感染率为10.03%,人乳头瘤病毒感染率为71.63%,生殖器癌/肛门癌病变发生率为0.7%。HIV阳性者高危型HPV感染率高（χ2=9.107,P=0.003）,OR=4.178（1.547~11.288）,HIV与低危型HPV无统计学关联（χ2=1.714,P=0.190）。HIV阳性者HPV感染基因型的分布与HIV阴性者不同（u=3.266,P=0.001）,R=0.192。另外,HIV阳性者高危型HPV感染种类数的分布也与HIV阴性者不同（u=3.740,P=0.000）,R=0.220。结论北京市男男性接触者人群HPV感染率、生殖器癌/肛门癌病变发生率较高,HIV与HPV基因型的危险度以及高危型HPV基因种类数相关,提示对HPV疫苗在MSM人群中的应用进行评价研究的必要性。OBJECTIVE To analyze relationship of HIV and anal HPV infection of MSM in Beijing,and to provide evidence for prevention of HIV and STDs. METHODS Recruited MSM voluntary through non-governmental organizations,interview questionnaire was applied,blood and anal swabs were collected for HIV-1 serological test and HPV genotyping. Analyzed data by χ2 test,rank sum test,rank correlation. RESULTS A total of 289 MSM were available. HIV and anal HPV prevalence were 10.03% and 71.63%,reported rate of genital cancer/anal cancer was 0.70%,high-risk HPV infection rate who had HIV-positive was higher than HIV-negative（χ2 = 9.107,P = 0.003）,OR = 4.178（1.547-11.288）. No statistically association was found between HIV and low-risk HPV（χ2 = 1.714,P = 0.190）. Genotypes distribution of HPV and number of high-risk HPV genotype in HIV-positive were different with HIV-negative（u = 3.266,P = 0.001,R = 0.192 and u = 3.740,P = 0.000,R = 0.220）. CONCLUSION Infection rate of HPV and incidence of genital cancer/anal cancer are high among MSM in Beijing,relationship is found between HIV and HPV genotypes,number of high-risk HPV genotype,which suggests that evaluation study on application of HPV vaccine is necessary.
