摘  要:目的探讨血管内皮生长因子(VEGF)表达在甲亢性心脏病(HHD)大鼠心室重构过程中的作用及缬沙坦对HHD大鼠心肌的保护作用。方法 SD大鼠32只随机均分为4组:A、B、C组腹腔注射L-甲状腺素0.5mg.kg-1.d-1×28d建立甲亢模型,B、C组同时分别用缬沙坦10mg.kg-1.d-1、30mg.kg-1.d-1灌胃;D组不用药,作为空白对照组。造模前后分别采取静脉血,用化学发光法检测血清三碘甲状腺原氨酸(T3)、甲状腺素(T4)水平;第28天行超声心动图检测左室舒张末期内径(LVEDD)、左室后壁厚度(LVPWT)、室间隔厚度(IVST)。完成心脏超声检查后,解剖动物测定各组大鼠心肌肥厚指数(MCHI)、左心室心肌细胞直径和胶原容积分数(CVF)及免疫组化观察心肌VEGF的表达。结果与D组比较,A、B、C三组血清T3和T4水平均明显升高,A组心肌VEGF表达明显增强,左室腔扩大,室壁增厚,MCHI、心肌细胞直径和CVF增大。而B、C组上述心室重构指标和VEGF表达的改变均比A组轻。结论 VEGF可能参与了HHD大鼠心肌重构过程;缬沙坦可抑制VEGF表达和心室重构,从而保护心功能。Objective To investigate the role of vascular endothelial growth factor (VEGF) expressed in rat heart with hyperthyroidism heart disease(HHD) on myocardial remodeling and the intervention effect of valsartan on it. Methods Thirty-two SD rats were equally randomized into 4 groups. The rat model of hyperthyroidism was established by daily intraperitoneal injection of L-thyroxine 0.5 mg.kg-1 .d-1 for 28 days in groups of A,B and C. At the same time,the rats in groups of B and C were given gastric gavage of valsartan 10 mg.kg-1 .d-1 and 30 rng.kg1 -d-1 , respectively. The rats in group D were taken as the blank controls. Serum triiodothyronine(T3) and thyroxine(T4) were examined before and after treatment. At the end of treatments, the left ventricular end-diastolic diameter(LVEDD), interventricular septal thickness (IVST) and left ventricular posterial wall thickness(LVPWT) were measured by echocardiography. The hearts were taken out for detecting the expression of VEGF by immunohistochemistry and measuring myoeardiac hypertrophy index(MCHI), left ventricular myocyte diameter(LVMCD) ,and collagen volume fraction(CVF). Results Compared to group D, serum levels of T3 and T4 in groups of A, B and C were remarkably elevated(P〈0. 01), the rats in group A showed significantly upregulated VEGF expression in the myocardium, and increased LVEDD, IVST, LVPWT, MCHI, LVMCD and CVF(P〈0. 01), which were less in groups of B and C than those in group A(P〈0. 01). Conclusion The VEGF expression in the myocardium participates in myocardial remodeling. Valsartan has a myocardial protection effect through downregulating VEGF expression in the myocardium and depressing myocardial remodeling.
