摘  要:目的探讨树突状细胞(DC)在变应性鼻炎患者外周血中的变化及对Th1/Th2细胞诱导分化的效应。方法用流式细胞术分析变应性鼻炎组(15例)、慢性鼻炎组(15例)及对照组(15例)患者外周血CD3-CD64+细胞和CD4+CD3-CD11c-细胞。将培养成熟的两种细胞分别与CD4+CD45RA+T细胞和抗CD3及抗CD28单抗共同培养,用ELISA法检测上清液细胞因子γ-干扰素(IFN-γ)、IL-4、IL-5和IL-10的含量。结果在变应性鼻炎组外周血中,以CD4+CD3-CD11c-细胞百分率增高为特征,经IL-3和CD40L等细胞因子培养后分化成熟成淋巴样细胞来源的树突状细胞(DC2);而慢性鼻炎组外周血中以CD3-CD64+细胞占优势,经用粒细胞集落因子(GM-CSF)+CD40L培养后分化成熟成髓样细胞来源的树突状细胞(DC1)。结论变应性鼻炎组外周血中DC的前体细胞(pDC1)/(pDC2)的失衡可能是导致变应性鼻炎患者外周血Th1/Th2失衡的重要原因之一,可能与变应性鼻炎发生密切相关。Objective To investigate the changes of dendritic cells(DC) and its effect on the differention of T-helper (Th)1/Th2 in the peripheral blood monocytes of patients with allergic rhinitis. Methods The CD4+ CD3+ CD11c- plasmacytoid cells (pDC2) and CD3- CD64+ cells (pDC1) in peripheral blood were analyzed with flow cytometry in 15 cases with allergic rhinitis(group A), 15 cases with chronic rhinitis (group B) and 15 healthy persons(group C). The mature DCs were cocultured with CD4+ CD45RA+ T-cells and antiCD3 and antiCD28 monoclonal antibody. The contents of IFN-T,IL-4,IL-5 and IL-10 in the supernatant were detected with ELISA. Results The increase of CD4+ CD3-CD11c- cells in group A was predominant, which differentiated into DC2 derived from mature T-cells after cocultured with IL-3 and CD40L. The increase of CD3- CD64+ cell in group B was predominant, which differentiated into DC1 derived from myeloid cells after cocultured with granulocyte macrophage colony-stimulating factor (GM-CSF)+ CD40L. Conclusion The imbalance of pDC1)/pDC2 may be one of the reasons for the imbalance of Thl/Th2, which may be closely related to the development of allergic rhinitis.
