摘  要:目的:初步探讨CT值分割技术在冠状动脉钙他眭斑块中的应用及价值。方法:分析186例患者CT冠状动脉钙化积分扫描图像的冠状动脉钙化(CAC),测量每一斑块的平均CT值、钙化积分(AS)和体积(CV),按照CT值范围将每一斑块分割为130-199Hu、200-299Hu、300-399Hu及≥400Hu四个组成部分,计算每一部分所占比率(%),以此代表每一斑块组成的差异,即每部分构成。使用Spearman等级相关进行变量间相关性分析,使用秩和检验进行样本间差别的显著性检验,P≤0.05为差异有统计学意义。结果:①每一钙化性斑块CT值分割后构成中除300-399Hu与≥400Hu这两部分之间差异无统计学意义外(P=0,19),其余各组成之间差别均有统计学意义(P值均小于0.001),且随着CT值的升高,各部分所占比率减少。②除CT值200-299Hu这部分组成以外其他各部分组成在不同性别及年龄组间差异均有统计学意义(P值均〈0.05)。③除CT值200-299Hu部分组成与每一钙化性斑块体积之间无显著相关性以外,各部分组成与CT值、AS及CV之间均具有相关性。结论:以CT值分割技术分析钙化性斑块的组成有助于对每一钙化性斑块内部构成的理解,能为研究钙化性斑块的稳定性及其进展和消退提供依据。Purpose: To preliminarily explore the significance of CT value segment technology in study of individual coronary artery calcified plaque. Methods: Coronary artery calcium (CAC) was analyzed in 186 successive participants of CT CAC score (CACS) examination. We determined the mean CT value, Agatston score (AS) and calcified volume (CV) of each individual calcified plaque in each subject, we also segmented each individual calcified plaque by CT value and obtained the constituent ratio of each segment, the segmental thressholds were 130-199Hu, 200-299Hu, 300-399Hu and ≥ 400Hu. Spearman rank correlation test was used to analyze the relevance between two variables and rank test was performed to analyze differential significance. P≤0.05 was considered to indicate statistical significance for all tests. Results: The proportion of low calcified component (130-199Hu) in individual calcified plaques was higher in male than in female, and the proportion of high calcified component (≥300Hu) was higher in female than in male. With the increasing of segmental CT threshold value, the correlation relevance between the constituent ratio of each segment segmented by CT value and other parameters were from negative to positive and more and more strong; and the constituent ratio of high CT threshold value segment was increased with the increasing of each parameter. Conclusion: The constituent ratio of each segment segmented by CT and its correlations with other parameters may be a useful method for studying the plaque stability and the progression and regression of CAC.
