自体膀胱黏膜尿道成形术治疗尿道下裂的临床体会

摘　　要：目的对自体膀胱黏膜尿道成形术治疗尿道下裂的疗效进行评价,对其适应证及手术方法进行讨论,以认识其应用价值。方法回顾性分析2004年1月至2010年6月在东南大学医学院附属江阴医院采用自体膀胱黏膜一期尿道成形术进行治疗的16例尿道下裂患儿的临床资料。结果术后患儿均获得较为满意的疗效,阴茎伸直满意,外形好,尿线粗,开口近似正常位置。无尿瘘发生。1例患儿于术后半年发生尿道口膀胱黏膜增生,小便分叉,予以二次手术切除尿道口多余黏膜外翻缝合后正常排尿。结论自体膀胱黏膜尿道成形术为治疗尿道下裂较理想的方法,此术式成功率高,外形效果良好。Objective To evaluate the outcome of bladder mucosa urethroplasty to treat hypospadias,discuss its indications and surgical methods to understand the application value.Methods The clinical data of 16 children with hypospadias from Jiangyin Hospital Affiliated to Southeast University Medical College r rom Jan.2004 to Jun.2010 who underwent bladder mucosa urethroplasty were analyzed.Results Sixteen cases were repaired successfully,penis straightness was satisfied with good shape,crude urinary and opening at approximate normal position.No urethral fistulae happened.One case developed mucosa hyperplasia six months after operation,which was repaired successfully with removing excess mucosa.Conclusion The bladder mucosa graft urethroplasty is an ideal procedure for hypospadias.The operation has many advantages such as beautiful shape and high cure rate.
