母婴同室新生儿医院感染危险因素分析及对策

摘　　要：目的分析母婴同室的新生儿医院感染危险因素,探讨降低其医院感染发生率的对策。方法采用回顾性调查方法,对2007年1月-2011年12月牡丹江医学院第二附属医院母婴同室新生儿的临床资料进行统计,分析其医院感染的特点、危险因素。结果母婴同室新生儿医院感染发生率为1.24%,其中2007～2011年感染发生率依次为3.17%、1.76%、0.74%、0.46%、0.28%;感染部位以呼吸道居首,占49.25%,其次为胃肠道感染、皮肤及脐部感染;感染的病原菌以表皮葡萄球菌为主,占24.24%,其次为肺炎克雷伯菌、大肠埃希菌等,占18.18%、16.67%;危险因素与新生儿自身易感、母婴同室环境、医疗护理操作、产妇及新生儿用具、医务人员的手等有关。结论加强母婴同室新生儿医院感染管理和消毒隔离制度,是预防控制母婴同室新生儿医院感染的关键。Objective To investigate the hazards of nosocomial infection of the new born in mother-baby together chamber, and to explore the countermeasure of lowering infection incidence rate. Methods A review investigation method was adopted; from January 2007 to December 2011, new born＇ s clinical data was collected, the characteristics of infection and hazards were an- alyzed. Results The incidence rate of nosocomial infection of the new born in mother-baby together chamber was 1.24% , among the year of 2007 -2011 ,the infectious incidence rate were one by one in order 3.17% , 1.76% ,0.74% ,0.46% and 0.28% ;the infection part ranked firstly with breath pass,with the rate of 49.25 %, followed by gastraintestine, skin and umbilicus;the patho- gen was mainly with staphylococcus, accounting for 24.24%, followed by Klebsiella pneumonia and E. coli, accounting for 18.18% and 16.67%, respectively;the hazards were related with new born＇ s oneself susceptibility, the environment of mother- baby together chamber, medical practice, Pregnant woman and new born＇ s appliance, and medical personnel＇ s hands etc. Con- clusion The key to the prevention and control of the nosocomial infection from mother-baby together chamber was to strengthen the management of nosocomial infection from mother-baby together chamber and disinfection insulate system.
