高位结扎联合激光腔内治疗大隐静脉曲张的随机对照研究

摘　　要：目的比较高位结扎联合激光腔内治疗(EVLT)和传统手术治疗大隐静脉曲张的疗效。方法将大隐静脉曲张患者(单侧)200例随机分为两组,一组行传统剥脱术,另一组行高位结扎联合激光腔内治疗。对比两组患者手术时间、术中出血量、术后住院时间、切口数量及术后疼痛评分。结果激光组手术时间、术中出血量、术后住院期限、切口数量及疼痛评分均低于传统组,两组差异有统计学意义(P<0.05);两组术后并发症发生率、复发率及术后生活质量无明显差别。结论高位结扎联合激光腔内治疗是值得推广的安全简单的微创手术。Objective To compare the effect of high ligation （HL） combined endovenous laser therapy（EVLT） and the tradi- tional stripping surgery in the treatment of varicose greater saphenous veins （GSV）. Methods Total 200 cases of great saphenous varicose vein（unilateral） were randomly divided into two groups, One group underwent the traditional stripped surgery, another group underwent HL combined with EVLT. The operative time, blood loss, hospitalization duration, number of incision, pain score, recurrence rate and quality of life of patients were compared between the two groups. Results The operative time, blood loss, hospitalization duration, incision quantity and pain score were lower in HL combined EVLT group as compared to the traditional group, the differences between the two groups there was statistically significant （ P 〈 0.05 ） ; There was no obvious difference be- tween the two groups about the incidence of complications, recurrence rate and the quality of life. Conclusion HL combined with EVLT is a safe and minimally invasive surgery in treatment of varicose greater saphenous veins.
