实时三维超声成像技术评价扩张型心肌病患者左心室收缩功能

摘　　要：目的探讨应用实时三维超声成像技术评价扩张型心肌病(dilated cardiomyopathy,DCM)患者左心室收缩功能的临床价值。方法使用实时三维超声成像技术分别测量DCM组患者和正常志愿者左室舒张末期容积(EDV)、左室舒张末期内径(LVIDd)、球形指数(SpI)、左室舒张末期质量(EDMass)、左室射血分数(LVEF),对两组进行比较并分析上述各参数与左室射血分数(LVEF)的相关性。结果 DCM组的EDV、LVIDd、SpI均大于正常组,EDMass、LVEF均小于正常组,差异具有统计学意义(P<0.05)。EDV、LVIDd、SpI与LVEF呈显著负相关(r=-0.879,P<0.01;r=-0.908,P<0.01;r=-0.708,P<0.01)。而EDMass与LVEF相关性较差(r=0.348,P<0.05)。结论实时三维超声成像技术可以为DCM疾病的诊断、疗效评估以及预后判断提供全新的检测手段,无创、准确、重复性强,具有很大的临床价值。Objective To evaluate the systolic function of the left ventricle in patients with dilated cardiomyopathy （DCM） by using real-time three-dimensional echocardiography. Methods The end-diastolic volume（EDV） ,left ventricular internal dimen- sion at end-diastole （LVIDd）, spherical index（ SpI）, end-diastolic mass （EDMass） and ejection fraction of left ventricle（LVEF） of patients with DCM and healthy volunteers was analyzed via three-dimensional echocardiography respectively. And the difference in the indexes above was compared between the two groups. The correlation between parameters above and left ventrieular ejection fraction was analyzed as well. Results When compared with the healthy subjects, EDV, LVIDd and SpI of left ventricle was sig- nificantly higher in the patients with DCM,while EDMass and LVEF was significantly lower（ P 〈 0.01 for all）. EDV, LVIDd and SpI of left ventricle was negatively correlated with LVEF （ r = - 0. 879, P 〈 0.01 ; r = - 0. 908, P 〈 0.01 ; r = - 0. 708, P 〈 0. Ol ）. But EDMass of left ventricle had a weak correlation with LVEF（ r = 0. 348,P 〈 0.05 ）. Conclusion Real-time three-di- mensional echoeardiography can provide a noninvasive, accurate, repeated detection mean for the diagnosis of DCM and prognosis judgment of patients.
