基层医院难愈性伤口病人的护理管理实践

摘　　要：目的通过对难愈性伤口患者治疗过程管理,探索难愈性伤口患者最佳管理模式。方法选择2011年1月～12月难愈性伤口患者38例作为实验组,注重护理管理,采取建立电子档案、压疮小组会诊、上级医院技术指导等,创面以改良封闭式负压引流(Vacuum-Assisted Closure,VAC)为主,辅以现代敷料治疗,出院后建立社区联络网、上门服务、电话随访等方法。2010年1月～12月难愈性伤口患者45例作为对照组,就患者护理管理质量、治疗环节质量控制及出院后伤口延续性治疗管理质量进行比较。结果实验组护理管理质量、治疗环节控制效果及出院后伤口延续性治疗管理质量明显提高(P<0.05)。结论规范难愈性伤口患者治疗护理方法,重视治疗过程管理,是护理安全及促进创面愈合的重要保障。Objective Through managing the treatment of the refractory wound patients, explore the best mode of management to the refractory wound patients. Methods 38 refractory wound patients from January to December 2011 in our hospital were served as the experimental group, this group was focused on the nursing management, including the constructing of electronic ar- chives, pressure sore group consultation, technical guidance from the superior hospital etc. The treatments on the wound were mainly VAC, assisted by the modem dressing treatment, and constructing the community communication net, home service, tele- phone follow-up after leaving the hospital, etc. The control group was 45 refractory wound patients form January to December 2010. The comparison between these two groups focus on the quality of nursing management,the quality control during the treat- ment, and the quality of continue treatment management after the patients leave the hospital was conducted. Results The quality of nursing management was significantly improved in the experimental group, including the result of control in the treatment, and the quality of continue treatment management after the patients leave the hospital. The differences between them were significant （ P 〈 0.05 ）. Conclusion Standardizing the treatment and nursing of the refractory wound patients,paying more attention to the management during the treatment, were two important guarantees to the safety of nursin~ and oromotinz the healing, of the wmmd.
