慢性持续期支气管哮喘患者外周血白细胞介素17的表达及意义

摘　　要：目的探讨白细胞介素17(IL-17)在哮喘严重程度判定中的价值。方法检测99例慢性持续期哮喘患者(哮喘组,按病情又分为轻、中、重三组)和30名健康人(对照组)用力1秒呼气率(FEV1%),进行诱导痰炎症细胞分类计数,并用ELISA法检测外周血中IL-17水平。结果哮喘组FEV1%明显低于对照组[(87.60±6.90)%、(68.50%±4.60)%、(48.30±5.90)%vs.(99.80±5.40)%](P<0.01)。哮喘组外周血IL-17水平明显高于对照组[(38.27±2.35)pg/ml、(40.36±2.33)pg/ml、(77.45±4.01)pg/ml vs.(14.05±3.08)pg/ml](P<0.01)。与对照组比较,哮喘组诱导痰嗜酸性粒细胞和中性粒细胞比率均明显升高(P<0.01)。血清IL-17与FEV1%、嗜酸性粒细胞、中性粒细胞呈正相关(r值分别为0.68、0.38、0.77,P<0.01、P<0.05、P<0.01)。结论 IL-17通过募集炎症细胞,特别是中性粒细胞参与了哮喘气道炎症的发生;IL-17可作为哮喘患者临床病情判定的参考指标。Objective To investigate the value of serum interleukin-17（IL-17） in evaluating the severity of chronic persistent asthmatic （CPA） patients. Methods The ratio of forced expiratory volume in 1 second to vital capacity（FEV1 %） ,serum IL-17（by ELISA） and differential cell counts of induced sputum were observed in 99 CPA patients [group A, with mild（group A1）, moderate（group A2） and severe asthma（group A3）] and 30 healthy volunteers（group C）. Results FEVl was lower in groups of A1,A2 and A3 than that in group C [（87.60＋6. 90）%,（68. 50%±4. 60）% and （48. 30±5.90）% vs. （99.80±5.40） %]（P〈0. 01）. Serum IL-17 was higher in groups of A1,A2 and A3 than that in group C [（38. 27±2.35） pg/ml, （40.36±2.33） pg/ml and （77. 45±4. 01） pg/ml vs. （14. 05 !3. 08） pg/ml] （P〈0. 01）. Compared to group C, the percentages of eosinophils and neutrophils in the induced sputum were higher in group A than that in group C（P〈0. 01）. Serum level of IL-17 was positively correlated with FEV1, eosinophils and neutrophils （r= 0. 68, r= 0. 33, r= 0. 77, P〈0. 01, P〈0. 05,P〈0. 01）. Conclusion Serum IL-17 plays a role in asthmatic patient airway inflammation by mobilizing inflammation cells, especially by recruiting neutrophils and has a potential value in evaluating the severity of asthma.
