摘　　要：目的:探讨高血压性心脏病(HHD)猝死者心肌连接蛋白43(Cx43)表达及其与猝死的关系。方法:挑选我校基础医学院法医学教研室2009-2011年尸体解剖案例20例,分为两组,每组各10例。①HHD组:男性9例,女性1例;年龄为41-73岁,平均年龄56.5岁;均为高血压性心脏病猝死。②非心源性死亡对照组:男性7例,女性3例;年龄为40-53岁,平均年龄46.8岁。每例心脏进行常规病理学检查并取材后,运用免疫组织化学和图像分析技术,检测左心室肌的Cx43染色情况。结果:HHD组心肌Cx43阳性颗粒数量显著减少,着色明显减弱,部分呈颗粒状,且分布于心室肌细胞内或细胞侧-侧连接处;对照组的染色未见明显变化。定量检测并经统计分析发现,HHD组Cx43蛋白染色阳性的面积和平均光密度与对照组均有非常显著的差异(P<0.01)。结论:高血压性心脏病猝死者心室肌Cx43蛋白的表达明显减少,高血压性心脏病的猝死可能与Cx43的表达变化有一定的关系。Objective： To investigate the relationship between the expression of connexin 43（Cx43） in myocardial cell of the left ventricle for patient with hypertensive heart disease（HHD） who died suddenly. Methods： Twenty autopsy cases were selected from the Department of Forensic Medicine of Wuhan University during 2009-2011.The cases were divided into two groups（HHD group and control group） with 10 cases in each group.After conventional pathological examination,Cx43 in the left ventricular myocardium of each case was detected with immunohistochemical staining.Results： In HHD group,the Cx43 positive staining were found in the cytoplasm or side-side connection of ventricular myocardial cell,and the positive area and average optical density of Cx43 positive particles were significantly decreased as compared with that respectively of the control group（both P 0.01）.Conclusion： The expression of myocardial Cx43 is significantly decreased in sudden-death cases with hypertensive heart disease,which suggests the decreased expression of Cx43 may have relation with sudden death of hypertensive heart disease.
