摘  要:目的探讨上颈椎韧带结构中横向韧带(横韧带)及纵向韧带对维持寰枢椎稳定性的作用。方法新鲜成人完整上颈椎标本6具,在生物力学试验机上按序测定完整标本、横韧带切断、横韧带+纵向韧带切断状态下寰枢椎的前屈/后伸、左/右侧屈、轴向旋转幅度及前屈时的寰齿前间隙(atlanto-dentalinterval,ADI)。结果(1)横韧带切断后,载荷10N时平均ADI值、前屈、后伸角度与完整标本比较差异有统计学意义;载荷30-90N时两组差异无统计学意义;载荷100N时ADI值2.90mm、前屈12.53°、后伸13.89°,载荷150N时ADI值3.37mm、前屈16.07°、后伸16.39°,与完整标本比较差异有统计学意义。Objective To investigate the role of transverse and longitudinal ligaments of the cran- iovertebral junction area in maintaining atlantoaxial stability. Methods Six human cadaveric cervical spines with the occipital attached (C0-C3) specimens were harvested. Specimens were tested in the following se- quence: the intact specimens (Intact group), the resection of transverse ligaments (TAL group), and the resec- tion of longitudinal ligaments based on TAL group (TAL+LL group). Results 1) At the loading of 10 N, the average of ADI, flexion, and extension were 1.34 mm, 5.65° and 6.97° respectively, significant difference was found between the intact group and the TAL group. No significant difference was found between the two groups under the physiological loading (30-90 N). At the loading of 100 N and 150 N, the average of ADI, flexion and extension were 2.90 mm, 12.53°, 13.89° and 3.37 mm, 16.07°, 16.39° respectively.
