摘　　要：目的介绍男性乳腺发育的不同手术方式，探讨不同程度男性乳腺发育的最佳手术方式。方法临床收治37例男性乳腺发育患者。分为3组，组1：胸部主要成分为脂肪；组2：胸部主要成分为增生的纤维腺体组织；组3：患者的“乳房”大且下垂，皮肤松弛。对不同组别的患者采用4种手术方式，包括单纯脂肪抽吸术、单纯乳晕切口腺体切除术、脂肪抽吸与腺体切除结合、乳房切除乳头乳晕游离移植术。结果所有患者对手术效果感到满意，胸部回复到男性正常平整的胸廓，一些并发症如皮肤瘢痕和乳头、乳晕麻木亦可以接受。结论根据不同的男性乳腺发育类型选择不同的手术方式，可有效地治疗男性乳腺发育，缓解患者的心理压力。Objective To introduce different surgical treatment for gyncomastia at different grades. Methods 37 eases with gynecomastia were divided into three grades as： grade I with fat as main tissue, grade Ⅱwith proliferated fibro-gland as main tissue, grade Ⅲ with big and ptosis breasts and sagging skin. Different surgical methods were chosen according to the different grades of gyncomastia. These include liposuction, subareolar fibroglandular tissue removing, combined technique of the two methods, and breasts resection with free transplantation of nipple-areola complex. Results All patients were satisfied for the appearance of post-operative flat male chest. Complications, such as scar, numbness of nipple and areola were acceptable for them. Conclusions Different surgical methods should be chosen for the gyneeomastia at different grades. It can improve both the physical and psychological problems for patients.
