尿毒症患者血脂联素水平与血管硬化的相关性研究

摘　　要：目的:探讨肾脏病尿毒症患者血清血脂联素水平及其与血管硬化的相关性。方法:将29例尿毒症血液透析患者设为治疗组,20例肾功能正常的截肢患者设为对照组。分别测定血清血脂联素水平、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-ch)、载脂蛋白B(apoB)水平;分别取桡动脉和同等级别的上下肢动脉做组织学检查,观察官腔管壁的变化及血脂代谢情况并对比分析。结果:治疗组患者血清血脂联素水平明显低于对照组(P<0.01),均存在不同程度的动脉血管硬化。相关分析显示患者血清血脂联素水平与桡动脉内膜中层厚度(IMT)及管壁/管腔比值呈负相关。结论:尿毒症患者普遍存在血管硬化,硬化的轻重与血脂联素水平成反比,这可能和血脂联素与脂质代谢紊乱密切相关有关。Objective：To investigate the correlation between serum adiponectin level and vascular sclerosis in patients with uremia. Methods： 29 patients with hemedialysis as the treatment greup,20 patients with normal renal function amputees as the control group. Tested the level of serum adi-ponectin, total cholesterol （ TC ）, triglyceride （ TG）, low density lipopretein cholesterol （ LDL - ch ）, apolipopretein B （ apo B ） ; at the same time talced the radial artery and the same level of upper and lower extremity artery for histological examination, observed the transformation of tube cavity wall and lipid metabolism. Results：The serum adiponectin levels of treatment group patients were significantly lower than control group （ P 〈 0.01 ）, uremic patients have different degree of arterial sclerosis. Correlation analysis showed that patients of serum adiponectin levels with radial artery intima - media thickness （IMT） and the wall/lumen ratio was negatively correlated. Conclusion：In uremic patients with widespread vascular sclerosis, hardening degree and adiponectin levels inversely,it might be due to adiponeetin and lipid metabolic disorder closely related.
