自行脱落单J管在腹腔镜胆总管一期缝合术中的应用

摘　　要：目的:探讨自行脱落单J管在腹腔镜胆总管一期缝合术中的应用价值。方法:回顾分析2008年1月至2012年11月30例胆总管结石择期手术患者的临床资料。腹腔镜下胆总管切开取石后,于胆总管内放置自行脱落单J管,再行胆总管一期缝合术。观察术后并发症、平均住院时间等相关指标。结果:30例手术均顺利完成,无胆漏等并发症发生。术后2～4 d恢复饮食,平均住院(9.3±1.8)d。结论:选择合适的病例,使用自行脱落J形管可为腹腔镜胆总管一期缝合提供安全保证,患者创伤小、术后康复快,值得进一步推广应用。Objective： To investigate the application value of self-releasing single-J tube in laparoscopic common bile duct surgeries with primary closure. Methods ： The clinical data of 30 patients undergoing selective operations of common bile duct stones or common bile duct stones complicated with gallstones from Jan. 2008 to Nov. 2012 were retrospectively analyzed. After laparoscopic choledoeholithotomy,a self-releasing single-J tube was placed in the common bile duct, and then the primary suture of common bile duct was completed. Postoperative complications and average length of hospital stay were observed. Results ： Thirty patients were all operated under laparoscope, no serious bile leakage or other complications occurred. The postoperative diet resumed after 2-4 days;the average length of hospital stay was （9.3 ±1.8 ） d. Conclusions ： Using the self-releasing J-shaped tubes can provide appropriately selected patients with security guarantees,little trauma and quick recovery for laparoscopie common bile duct surgeries with primary suture, and it deserves further popularization and application.
