单通道经皮肾镜超声碎石治疗鹿角形结石的临床研究

摘　　要：目的:探讨并总结单通道经皮肾镜超声碎石治疗鹿角形结石的临床效果。方法:本文选取2007年10月-2012年10月来笔者所在医院接受手术治疗的鹿角形结石患者140例,对其临床资料进行回顾性总结,依据治疗方法,分观察组与对照组,各70例,观察组患者接受单通道经皮肾镜超声碎石治疗,对照组患者接受开放手术治疗,对两组患者输血百分比、手术时间、血红蛋白、术后并发症发生率以及住院时间、术后1个月结石的清除率、治疗费用进行比较总结。结果:与对照组相比,观察组在手术前后患者血红蛋白的下降量、输血率、术后并发症发生率、手术时间以及住院时间均较少,而观察组的治疗费用和手术后1个月患者结石清除率则显著高于对照组,两组比较差异有统计学意义(P<0.05)。结论:单通道经皮肾镜超声碎石治疗鹿角形结石安全可靠,但在治疗费用方面较开放手术治疗高。Objective： To explore the efficacy of single-channel percntaneous nephroscope lithotripsy （MPCNL）by ultrasonic in the treatment of renal staghorn calculi.Methods： 140 cases of renal staghorn calculi in our hospital from October 2007 to October 2012 were assigned to observation group and control group, and 70 cases in each, patients in observation group received MPCNL while others received open surgery.The operation time, total operative blood loss, blood transfusion rate, postoperative hospital stay, complication rate, total treatment cost and calculi clearance at one month of postoperative follow-up between two groups were evaluated.Results： Compared with open surgery, MPCNL was associated with significantly less drop of hemoglobin, less blood transfusion, shorter operation time, less postoperative hospital stay, less complication rate, but more total treatment cost, higher calculi clearance rate, which were significant differences between the two groups（P〈0.05）.Conclusion： MPCNL is a safe and effective way to treat renal staghorn calculi, though it has higher total medical cost.
