摘  要:目的对比观察封闭负压引流技术及削痂术对兔深Ⅱ度烫伤创面愈合及CK、bFGF表达的影响。方法选用新西兰兔15只制备深Ⅱ度烫伤模型共60个创面,于建模1天后分3,7,10,14,21天共5个时相点实施VSD治疗(VSD组,n=6)和削痂术(削痂组,n=6),采集创面皮肤组织样本,通过免疫组化染色对比观察两组创面组织bFGF和CK表达并进行统计学分析。同时连续观察测量创面愈合情况,计算创面愈合率。结果 VSD组第7、10、14天的bFGF表达高于削痂组(P<0.05);第7,10,14,21天的CK阳性表达强于削痂组;VSD组创面坏死组织清除和炎症消退早于削痂组,从第7天开始,创面愈合率高于削痂组(P<0.01),创面愈合时间缩短[(19.35±1.49)天vs(23.07±1.31)天,P<0.01];结论 VSD技术较削痂术能明显提高创面组织bFGF和CK的表达,加速兔深Ⅱ度创面愈合,缩短治疗时间,改善创面愈合质量。Objective To observe the different influence of VSD on the express of bFGF, CK and healing of wound comparing with tangential incision in the scald model of deep second skin. Methods Fifteen rabbits of New Zealand were se- lected and established a scald model of deep second degree skin on the back. It was divided into two groups of VSD treatment and tangential incision. The express of bFGF, CK were determined by immunohistochemistry stain and the wound healing rate was ob- served. Results The express of bFGF, CK in VSD group was significantly higher respectively than that of tangential incision group in the 7th, lOth , 14th day after injury (P 〈0. 05) . The wound healing time in VSD group was also shorter than that of tangential incision group [ (19.35 ±1.49) d v. s (23.07 ±1.31) d, P〈0. 01] . Conclusion The VSD could promote the wound healing and the mechanism was probably through the high express of bFGF, CK.
